三重带袢纽扣钢板与锁骨钩钢板治疗急性肩锁关节脱位的远期疗效

摘　　要：[目的]比较三重带袢纽扣钢板系统与锁骨钩钢板治疗RockwoodⅢ型肩锁关节脱位的远期疗效。[方法]2008年10月~2010年3月,分别对64例RockwoodⅢ型肩锁关节脱位的患者采用三重带袢纽扣钢板系统重建喙锁韧带与锁骨钩钢板治疗。对两组的并发症、治疗费用、术后第5年Constant评分进行对比分析。[结果]术后第5年时,三重带袢纽扣钢板系统组与锁骨钩钢板组的Constant评分分别为（95.69±5.40）分、（94.22±6.32）分,差异有统计学意义（P=0.001）;两组Constant评分的差异主要是外展活动评分（9.50±0.88、9.25±0.98,P=0.040）;锁骨钩钢板组有4例发生肩锁关节半脱位,三重带袢纽扣钢板1例发生纽扣钢板滑脱;两组的治疗费用分别为（20220.00±1160.17）元、（21720±768.30）元,差异有统计学意义（P=0.003）。[结论]RockwoodⅢ型肩锁关节脱位的远期疗效,三重带袢纽扣钢板系统优于锁骨钩钢板,治疗费用更低。[ Objective ] To evaluate and compare the long - term clinical outcomes of triple endobutton plates versus clavic- ular hook plate for the treatment of Rockwood type 11I acromioclavicular joint dislocation. [ Method ] From October ：2008 to March 2010,64 patients with Rockwood type m acromioclavicular joint dislocation were treated with triple endobutton plates or clavicular hook plate, respectively. The postoperative complications, cost of treatment, the Constant scoresat the fifth year after the operation were recorded and statistically compared between two groups. [ Result ] At the postoperative fifth year, the mean Constant score of the triple endobuttan plates group was 95.69 ± 5.40, and 94.22 ± 6.32 for the clavicular hook plate group. There was a statistically significant difference between the two groups （ P = 0. 001 ）, the most contributed factor for the difference was the muscle strength at 90°lateral extension（9.50 ±0.88 vs 9.25 ± 0. 98, P = 0. 040 ）. In the triple endobutton plates group, there was oneplate detachment. While in the clavicular hook plate group, four cases of acromioclavicular joint sublux- ation were found. The cost of treatment also showed statistical- ly significant difference （triple endobutton plates group 20220.00 ± 1160.17 yuan, clavicular hook plate group 21720± 768.30 yuan, P 〈 0.05 ）. [ Conclusion ] The long - term effiect of triple endobutton plates is better than clavicular hook plate, moreover,it needs less hospital costs.
