摘  要:目的:探讨低分子肝素(LMWH)对重症急性胰腺炎(SAP)的临床疗效及可能机制。方法:采用前瞻性研究,将126例SAP患者分为普通治疗组(A组)和普通治疗+LMWH治疗组(B组),治疗2周后对比两组患者血清NO浓度、APACHⅡ评分、临床症状改善程度、胰腺Balthazar CT分级。结果:与A组相比,治疗2周后B组血清NO浓度显著升高(P<0.01)、APACHⅡ评分显著降低(P<0.01),临床症状显著改善(P<0.01),胰腺Balthazar CT分级显著降低(P<0.01)。结论:LMWH可能通过增加NO合成,利用其扩血管作用增加胰腺血流量,改善胰腺微循环障碍,发挥辅助性治疗作用,改善SAP转归。Objective: To investigate the clinical efficacy of low molecular weight heparin (LMWH) in the treatment of acute severe pancreatitis(SAP) and possible mechanisms. Methods: A prospective analysis of 126 cases of severe SAP patients,they were divided into conventional therapy group(groupA, n=61) and and conventional therapy group plus low molecular weight heparin therapy group(group B, n=65), Comparative analysis of treatment the two groups of serum NO concentration, APACH Ⅱ score, clinical symptom improvement, Pancreatic Balthazar CT grading. Re. suits: Compared with group A, B group NO levels was significantly increased (P〈0.01),APACH II score was significantly Iower(P〈0.05), clinical symptoms improved significantly(P〈0.01 ), Pancreatic Balthazar CT grading significantly reduced (P〈0.01). Conclusion: LMWH possible by increasing serum NO concentration, leading to pancreatic tissue blood vessels to dilate, thereby improve pancreatic microcirculation.
