2014年天津市医疗机构传染病疫情报告管理现况研究

摘　　要：目的了解天津市医疗机构传染病疫情报告质量和管理现状,发现各级医疗机构在传染病报告工作中存在问题,为进一步优化该市传染病疫情报告管理流程提供理论依据。方法采用分层随机方法抽取天津市16个区县（滨海新区塘沽、汉沽、大港分别抽取）二级及以上医疗机构18家,一级医疗机构36家开展调查。结果共抽查传染病634例,传染病报告卡605张。医疗机构传染病平均报告率为95.4%（95%CI：93.7%-97.0%）,不同级别的医疗机构报告率差异无统计学意义。门诊病例与住院病例的报告率差异无统计学意义。一级医疗机构的报告及时率、纸质报告卡填写准确率、网络报告信息一致率高于二级及以上医疗机构。结论天津市医疗机构传染病有漏报现象,医疗机构传染病疫情报告质量亟待提高。应加强对医疗机构的督导和培训工作,提高医疗机构对传染病疫情报告工作的重视程度,优化电子病历系统下传染病疫情报告管理的工作流程。Objectives To understand the quality and management status of communicable disease reporting in medical institutions of Tianjin City,and to identify the existing problems and provide theoretical basis for further optimization of reporting process in local area.Methods Random method was adopted to select 18 medical institutions of grade 2and above and 36 of grade 1from 16 counties and districts（each selection was performed in Tanggu,Hangu and Dagang of Binhai New Area）for investigation.Results A total of 634 cases were selected,and 605 reporting cards were checked.The rate of communicable report was 95.4%（95%CI93.7% -97.0%）in average.The rate ofcommunicable report was not significantly different among medical institutions of different level,and neither between outpatients and inpatients.The rates of timely report,accuracy of paper card,consistent rate of information in grade 1medical institutions were higher than those of grade 2and above.Conclusions The missing report of disease exists in medical institutions of Tianjin city,and the quality of reporting needs to be improved.It is necessary to strengthen supervision and training,to pay more attention to disease reporting,to improve reporting process of communicable disease by using digital management system of medical record in medical institutions of all levels.
