摘  要:目的探讨N-乙酰半胱氨酸(NAC)对坏死性小肠结肠炎(NEC)SD新生大鼠模型肠道损伤的保护作用。方法 3日龄SD大鼠40只随机分为4组:NEC模型组、NAC对照组、生理盐水对照组、NEC模型+NAC组,每组10只。通过缺氧复氧冷刺激5 d制备NEC模型,此后,除NEC模型组外,其余3组给予NAC(100 mg/次)或生理盐水(5 ml/次)灌胃,1次/d,共5 d。各组在实验第5、10 d分别处死5只大鼠,解剖回盲部近端肠管行病理学检查,静脉血测定血清丙二醛(MDA)、谷胱甘肽-过氧化物酶(GSH-PX)和超氧化物酶(SOD)水平。结果 NEC模型组实验中共死亡2只,其余组无死亡。NEC模型+NAC组和NEC模型组第10 d肠道组织病理评分均高于生理盐水对照组和NAC对照组(P<0.05),NEC模型+NAC组评分低于NEC模型组(P<0.05)。在实验后第5 d及第10 d,NEC模型组血清抗氧化活显著低于NAC对照组、生理盐水对照组(P<0.05),也显著低于NEC模型+NAC组(P<0.05),提示NAC可明显提高血清抗氧化活性(P<0.05)。结论 NAC可以减轻NEC大鼠模型肠道组织病理损伤,提高机体抗氧化水平,具有良好的肠道保护作用。Objective To explore the protective effects of N-acetylcysteine (NAC) against the intestinal injury in SD neonatal rats with necrotizing enteroeolitis (NEC) model. Methods A total of 40 SD rats aged three days were randomly divided into four groups: NEC model group, NAC control group, normal saline group, and NEC model + NAC group (n = 10, respectively). NEC models were prepared by means of hypoxia and re-oxygenation cold stimulation for five days. Therefore, except for NEC model group, the other three groups received lavage with NAC(100 mg/time) or normal saline(5 ml/time), once a day for total five days. Five rats in each group were killed on the fifth and tenth day of the experiment respectively. The anatomy of the ileoceeal valve in the proximal intestine was conducted for pathological examination. The levels of serum MDA, GSH-PX and SOD were detected via the venous blood. Results Two rats in NEC model groups died in the experiment. No rat died in other groups. The intestinal tissue pathology scores on the 10th day in NEC model+NAC group and NEC group were higher than those in the normal saline control group and NAC control group(P 〈 0.05). The scores in NEC model+NAC group were lower than those in NEC model group (P 〈 0.05). On the fifth and tenth day of the experiments, the serum antioxidant activity in NEC model group was much lower than that in NAC control group and normal saline group (P 〈 0.05), and significantly lower than that in NEC model+NAC group (P 〈 0.05), indicating that NAC could significantly improve serum antioxidant activity (P 〈 0.05). Conclusion NAC can mitigate the intestinal histologic injury in NEC rats and increase the antioxidative level, and can provide excellent intestinal protection.
