摘  要:目的探讨胆汁外引流对梗阻性黄疸大鼠肠紧密连接和水通道蛋白8(AQP8)表达的影响。方法将30只SD大鼠随机分为假手术对照组(S组)、梗阻性黄疸模型组(OJ组)和梗阻性黄疸外引流组(ED组),每组各10只。OJ组建立梗阻性黄疸模型,ED组于建模后第7天插入引流管;各组动物于手术后第7天,取肠组织,一部分固定于中性甲醛溶液中,常规组织切片、HE染色后观察;另一部分冻于液氮,Western-blot法检测ZOs蛋白、闭锁蛋白(Occludin)和AQP8蛋白表达情况。结果与S组比较,OJ组大鼠肠黏膜完整破坏,组织连接断续,黏膜发生萎缩、糜烂或溃疡改变。ED组大鼠黏膜排列相对规则,黏膜萎缩、糜烂或溃疡改变有所恢复;肠绒毛高度/宽度比值结果显示,OJ组和ED组肠绒毛均有所下降(P<0.05);与OJ组比较,ED组肠绒毛有所升高(P<0.05)。Western-blot结果显示,S组ZOs蛋白、Occludin蛋白和AQP8蛋白表达最高,与OJ组和ED组比较,差异均有统计学意义(P<0.05);而且,ED组高于OJ组(P<0.05)。结论黄疸可影响肠屏障功能,降低黏膜ZOs蛋白、Occludin蛋白和AQP8蛋白表达。胆汁外引流法通过提高黏膜ZOs蛋白、Occludin蛋白和AQP8蛋白表达,对损伤的肠道具有保护作用。Objective To explore external bile drainage of obstructive jaundice rat intestinal tight connection and the influence of aquaporin 8( AQP8) expression. Methods A total of 30 SD rats were randomly divided into the sham operation control group( Group S),the model of obstructive jaundice group( Group OJ) and the external drainage of obstructive jaundice group( Group ED),with10 rats in each group. A model of obstructive jaundice was established in Group OJ,drainage tube was inserted within 7 days after modeling in Group ED; 7 days after surgery in each animal,intestinal tissues were extracted,part of the tissues were fixed in neutral formaldehyde solution,normal tissue biopsies,and observation was carried out after HE staining; the other part of the tissues were frozen in liquid nitrogen,western blot was used to detect ZOs,occludin and AQP8 protein expression. Results Compared with Group S,intestinal mucosa was completely destructed,organization connection was interrupted,erosion or ulcer change occurred in Group OJ; in Group ED,intestinal mucosa was in arrangement rules,and mucosal atrophy,erosion or ulcer change recovered; the intestinal villus height / width ratio was reduced according to the results in Group OJ and Group ED( P 0. 05); compared with Group OJ,intestinal villus of Group ED was increased( P 0. 05). According to the results of western blot,ZOs,occludin,and AQP8 protein expression were the highest in Group S,compared with those in Group OJ and Group ED,and there was significant difference( P 0. 05); further more,expression in Group ED was higher than that in Group OJ( P 0. 05). Conclusion Jaundice can affect the intestinal barrier function,reduce ZOs,occludin and AQP8 protein expression; bile extra drainage method can promote ZOs,occludin and AQP8 protein expression to protect the injured intestinal.
