摘  要:目的通过调查了解母婴分离产妇的泌乳状况及泌乳不足的高危影响因素。方法选择2014年7月至2015年6月在上海市第一妇婴保健院分娩,其婴儿并被收治入新生儿科的单胎新生儿母亲270名。采用上海市第一妇婴保健院自行设计的吸乳日记,调查对象每日记录吸乳的时间、乳汁量以及新生儿乳汁需要量,对数据进行统计分析。结果调查对象平均记录天数为13.4±14.3天,平均泌乳量为每天571.5±353.4m L。母亲日泌乳量不能满足当日婴儿需求占20.5%。母亲泌乳不足的高危因素依次是早产(OR=13.83)、每日吸乳不满8次(OR=3.24)、文化程度低(OR=1.96)、初产妇(OR=1.72)和剖宫产(OR=1.28),(均P<0.05)。结论医护人员应准确识别存在泌乳不足高危因素的产妇,及时给予有效的干预措施,以促进住院新生儿的母乳喂养。Objective To know lactation condition of puerpera separated with their infants and high risk factors causing insufficient lactation through investigation .Methods Altogether 270 cases of singleton neonatal mothers who delivered and whose infants were hospitalized in Shanghai First Maternity and Infant Hospital from July 2014 to June 2015 were selected .Self-designed“milk secretion journal” was used to get information.Suction time, maternal lactation, and infants’ daily requirement for breast milk were recorded by respondents .Recorded data was analyzed .Results Average number of days for recording “milk secretion journal” was 13.4 ±14.3, and average daily lactation was 571.5 ±353.4ml.The percentage of insufficient milk supply was 20.5%.High risk factors causing insufficient milk production were premature delivery (OR=13.83), less than 8 times of milk sucking per day (OR=3.24), lower education level of mother (OR=1.96), primipara (OR=1.72) and caesarean section (OR=1.28) (all P〈0.05).Conclusion Medical workers should accurately identify puerperae with high risk factors for insufficient lactation and conduct timely intervention in order to promote breastfeeding in hospitalized infants.
