摘 要:目的:调查暑期儿童血清25-羟维生素D[25(OH)D]水平,为合理补充维生素D提供科学依据。方法选取2015年7-8月间在泸州医学院附属医院儿童保健科门诊常规体检的1~16岁儿童670名为研究对象,采用化学发光法检测儿童血清25(OH)D,以体质量指数(BMI)将研究对象分为肥胖组、超重组及正常组。结果儿童25(OH)D总体水平为28.48±10.40 ng/mL,缺乏及不足率高达60.30%,>7~16岁组、1~3岁组及>3~7岁组儿童25(OH)D水平差异具有统计学意义,其中1~3岁组最高,>7~16岁组最低,组间比较有统计学意义(F=77.218,P<0.05)。肥胖组、超重组儿童25(OH)D水平缺乏率明显高于正常儿童组(χ2值分别为6.94、7.12,均P<0.05);城区儿童25(OH)D水平明显低于农村儿童(t=-9.128,P<0.05)。 BMI与25(OH)D呈负相关(r=-0.242,P<0.05)。结论泸州市儿童血清25(OH)D总体不足;儿童BMI与25-羟维生素D呈负相关;城区儿童血清25(OH)D总体水平偏低。应加强血清25(OH)D检测,尤其肥胖儿童及城区居住儿童,积极予以合理的维生素D补充并加强对此类儿童的随访观察。Objective To investigate the serum 25-hydroxy vitamin D [25(OH)D] level of children in summer vacation so as to provide scientific basis for supplementation of vitamin D .Methods Totally 670 children aged from 1 to 16 years were selected during July to August in 2015, and all cases came from the Affiliated Hospital of Luzhou Medical College , who took routine examination in the department of child health care.The serum 25(OH)D levels were measured by electrochemiluminescence .All cases were divided into overweight group , obesity group or normal group by body mass index (BMI).Results The overall level of 25(OH)D was 28.48 ±10.40ng/mL, and the rate of deficiency and insufficiency reached 60.30%.The serum 25(OH)D level was statistically different among group over 7, group aged under 3 and group aged 3-7.It was highest in group under 3 and lowest in group over 7 with statistical significance (F=77.218, P〈0.05).The 25(OH)D deficiency rate in obesity group and overweight group was obviously higher than that in normal group (χ2 value was 6.94 and 7.12, respectively, both P〈0.05).The serum 25(OH)D levels of urban children were lower than those of rural children (t=-9.128, P〈0.05).BMI showed negative correlation with serum 25(OH)D (r =-0.242, P 〈0.05).Conclusion The overall insufficiency of serum 25( OH) D exists in children in summer vacation in Luzhou city , and BMI is negatively correlated with serum 25 ( OH) D.The levels of 25-( OH) D are lower in urban children .It suggests that monitoring 25-( OH) D should be reinforced , especially for obesity children and urban children , scientific measure should be taken to supplement vitamin D intake and enhance follow -up.
