摘 要:目的探讨B型柯萨奇病毒性心肌炎(CoxB-VMC)患者血清白细胞介素-6(IL-6)、白细胞介素-17(IL-17)、白细胞介素-18(IL-18)及单核细胞趋化蛋白-1(MCP-1)水平的变化及联合检测对CoxB-VMC临床诊断的价值。方法选取VMC患者158例为实验组,健康体检者94例为对照组。采用酶联免疫吸附法(ELISA)测定两组患者血清IL-6、IL-17、IL-18、MCP-1的含量及B型柯萨奇病毒(CoxB)IgM抗体,电化学发光法检测两组血清中肌酸激酶同工酶(CK-MB)及心肌肌钙蛋白T(cTnT)的水平,分析各指标的水平与CoxB-VMC的关系,评价IL-6、IL-17、IL-18及MCP-1联合检测对CoxB-VMC诊断的价值。结果AVMC急性期血清CK-MB、cTnT、IL-6、IL-17、IL-18及MCP-1水平分别为(27.90±4.72)、(0.37±0.02)、(361.60±45.39)、(445.70±67.97)、(168.60±35.87)、(376.20±49.50)ng/L,显著高于对照组(P〈0.01);VMC患者CoxB病毒感染与心脏功能分级、IL-6、IL-17、IL-18及MCP-1水平呈正相关(P〈0.05)。IL-6、IL-17、IL-18及MCP-1联合检测可显著提高CoxB-VMC检测的敏感性(97.30%)、特异性(94.60%)及准确率(96.70%)。结论VMC急性期IL-6、IL-17、IL-18及MCP-1水平变化在一定程度上可反映CoxB-VMC的心脏功能状态,该4种指标的联合检测可增强CoxB-VMC的临床诊断和病情评估。Objective To determine the levels of serum IL-6, IL-17, IL-18 and MCP-1 and the diagnostic value of the combination detection of IL-6, IL-17, IL-18 and MCP-1 on CoxB-VMC. Methods The 158 patients with VMC were selected as the experiment group and 94 healthy persons were selected as the control group. The serum CK-MB and cTnT were tested by electrochemolumines- cence assay, while the levels of IL-6, IL-17, IL-1, MCP-1 and CoxB IgM antibody were detected by ELISA assay. Results The serum levels of CK-MB, cTnT, IL-6, IL-17, IL-18 and MCP-1 in patients with VMC were (27. 90±4. 72), (0. 37±0. 02), (361.60±45.39), (445.70±67.97), ( 168.60 ±35.87 ) and (376.20 ~ 49.50) ng/L respectively, which were significantly higher than those of healthy persons (P 〈0.01 ). The CoxB virus infection in VMC patients was positively correlated with tlae heart function classification, IL-17, IL-18, IL-6 and MCP-1 levels (P 〈 0. 05 ). The combined detection of IL-6, IL-17, IL-18 and MCP-1 can significantly improve the sensitivity (97.30%), specificity (94.60%) and accuracy (96.70%)of CoxB-VMC detection. Conclusions The serum levels of IL-6, IL-17, IL-18 and MCP-1 in acute phase can reflect the cardiac function of patients with CoxB-VMC in certain extent, and the combined detection of these four indicators can effectively enhance the clinical di- agnosis and assessment of CoxB-VMC.
