血栓通冻干粉针治疗急性脑梗死临床疗效及对患者血浆TNF-α、Hcy、Fib水平的影响

摘　　要：目的探讨血栓通冻干粉针治疗急性脑梗死的临床疗效及对患者血浆同型半胱氨酸(Hcy)、肿瘤坏死因子-α(TNF-α)与纤维蛋白原(Fib)水平影响。方法将80例急性脑梗死患者随机分为两组。对照组采用常规治疗,治疗组在对照组治疗基础上加用血栓通冻干粉针。两组患者均连续治疗2周,比较分析两组临床疗效、神经功能缺损评分及不良反应发生率,酶联免疫吸附(ELISA)法测定治疗前后血浆TNF-α、Hcy、Fib水平。结果治疗组总有效率显著高于对照组(P<0.05),治疗后神经功能缺损评分治疗组明显低于对照组(P<0.05),治疗后两组血浆TNF-α、Hcy、Fib水平均降低(P<0.05),治疗组治疗后血浆TNF-α、Hcy、Fib水平比对照组降低更明显(P<0.05),两组不良反应发生率比较差异无统计学意义(P>0.05)。结论血栓通冻干粉针治疗急性脑梗死临床疗效较好,可显著改善患者神经功能缺损症状,降低血浆TNF-α、Hcy、Fib水平。Objective To investigate the efficacy of Xueshuantong lyophilized powder in treatment of acute cerebral infarction and its effects on levels of plasma homoeysteine （Hcy） , tumor necrosis factor - α （ TNF - α） and fibrinogen （Fib）. Methods 80 patients with acute cerebral infarction were randomly divided into control group and observation group, 40 cases in each group. Both groups were treated for 2 consecutive weeks. Clinical efficacies, neurological deficit scores and incidence of adverse reactions were comparative analyzed. Plasma TNF -α, Hey, Fib levels before and after treatment was detected by ELISA assay. Results The total effective rate of observation group was 90. 0% , significantly higher than 72% of control group （P 〈 0. 05 ）. After treatment, neurological deficit scores of observation group were significantly lower than those of control group （ P 〈 0.05 ）. Levels of plasma TNF -α, Hcy, Fib levels in both groups reduced after treatment. Compared with control group, levels of plasma TNF - α, Hey, Fib in observation group were significantly lower （ P 〈 0. 05 ） , the difference was statistically significant. The incidence of adverse reactions in the two groups showed no significant difference （ P 〉 0.05）. Conclusion Xueshuantong lyophilized powder is effective in treatment of acute cerebral infarction, it can significantly improve the symptoms in patients with neurological impairment and decrease the levels of plasma TNF -α, Hcy, Fib.
