脓毒症患者超声心动图功能指标与白细胞介素-6、白细胞介素-10、肌钙蛋白T、可溶性晚期糖基化终末产物受体的关系

摘　　要：目的探讨脓毒症患者超声心动图心功能指标与患者血清白细胞介素-6(IL-6)、白细胞介素-10(IL-10)、肌钙蛋白T(cTnT)、血浆可溶性晚期糖基化终末产物受体(sRAGE)水平的关系。方法选取92例脓毒症患者,测定患者入院24小时内的血清IL-6、IL-10、cTnT、血浆sRAGE水平,根据左心室射血分数(LVEF)分为心功能正常组41例(LVEF≥50%)、心功能异常组51例(LVEF<50%),并于入院后72小时内行超声心动图检查,分析心功能指标与血清IL-6、IL-10、cTnT、血浆sRAGE的关系。结果正常组患者LVEF、E/A测定值显著高于异常组,差异有显著性(P<0.05);正常组患者左心室舒张末期内径、心率测定值显著低于异常组患者,差异有显著性(P<0.05);正常组患者血清IL-6、IL-10、cTnT、血浆sRAGE测定值显著低于异常组患者,差异有显著性(P<0.05);脓毒症患者血清cTnT、血浆sRAGE与LVEDD比较,差异有显著性(P<0.05),血清IL-6、cTnT、血浆sRAGE测定值与LVEF值比较,差异有显著性(P<0.05)。结论脓毒症患者炎症反应及心肌损伤导致患者左心室功能发生改变。Objective The relationship of sepsis by ultrasound in patients with cardiac figure index of heart function and serum leukocyte mediated IL-6（IL-6）and leukocyte mediated IL-10（IL-10）,troponin T（cTnT）,plasma soluble advanced glycation end product receptor（sRAGE） level.Method Select 92 cases of patients with sepsis,admission measured serum IL-6,IL-10,cTnT,plasma levels of sRAGE within 24 h,according to the left ventricular ejection fraction（LVEF）points for cardiac function in normal group（41 cases）（LVEF≥50%）and cardiac function in abnormal group 51 cases（LVEF50%） and in a hospital after 72 hours of expert ultrasound heartbeat graph examination.Analysis the relationship between heart function and serum IL-6,IL-10,cTnT,plasma levels of sRAGE.Result The normal group of patients with echocardiographic indexes of LVEF,E/A values significantly higher than abnormal group,the difference was statistically significant P〈0.05）;normal group patients with echocardiographic indexes of LVEDD,HR was significantly lower than that of patients with abnormal group,the difference was statistically significant（P〈0.05）;group IL-6,patients with normal serum IL-10,cTnT,plasma sRAGE were significantly lower than the abnormal group patients,the difference was statistically significant（P〈0.05）;patients with sepsis serum cTnT and plasma sRAGE of patients with sepsis LVEDD showed a significant positive correlation（P〈0.05）,serum IL-6,plasma cTnT,sRAGE value was negative a significant correlation with LVEF value（P〈0.05）.Conclusion Changes of left ventricular function in patients with sepsis induced by inflammatory reaction and myocardial injury.
