VVI评价妊娠高血压综合征孕妇左心室收缩功能

摘　　要：目的应用速度向量成像技术（VVI）检测中晚孕期正常妊娠者和妊娠期高血压综合征（妊高征）病人左心室收缩功能。方法选择30例正常中晚期妊娠（20~38周）妇女作为正常妊娠组,26例中晚期妊娠的妊高征妇女作为妊高征组,另选择健康未孕妇女30例作为对照组。应用VVI分别测量各组左心室整体收缩期纵向峰值速度、峰值应变、峰值应变率。结果与对照组比较,正常妊娠组收缩期纵向峰值速度未见明显改变（P〉0.05）,收缩期纵向峰值应变和应变率均减低,差异有显著性（F=28.43~44.24,q=6.76~8.92,P〈0.01）;妊高征组收缩期纵向峰值速度、峰值应变和应变率均明显减低,差异有显著性（F=3.36~44.24,q=3.61~12.94,P〈0.05）。与正常妊娠组比较,妊高征组收缩期纵向峰值速度未见明显差异（P〉0.05）,但纵向峰值应变、峰值应变率均减低,差异有统计学意义（q=3.97~4.35,P〈0.01）。结论妊高征妇女左心室心肌纵向收缩功能受损,VVI能够显示正常妊娠及妊高征孕妇左心室收缩功能的早期损伤。Objective To evaluate the left ventricular systolic function（LVSF）in normal pregnant women and patients with pregnancy-induced hypertension syndrome（PIHS）in their middle and late pregnancy（MLP）using velocity vector imaging（VVI）. Methods Thirty normal MLP（20-30-week gestation）women were selected and served as normal pregnancy group,26 MLP with PIHS served as PIHS group,and 30 healthy non-pregnant served as control group.Employing VVI,the global longitudinal peak systolic velocity（Vpeak）,the global longitudinal peak strain（GLPS）and the peak strain rate of the left ventricle were measured. Results Compared with the control group,there was no significant changes in the Vpeak in normal pregnancy group（P〉0.05）,while the GLPS and the strain rate were significantly lower（F=44.24,28.43;q=8.92,6.76;P〈0.01）.The Vpeak,the GLPS and the strain rate of the patients in the PIHS group significantly decreased（F=3.36-44.24,q=3.61-12.94,P〈0.05）.Compared with the normal pregnancy group,there was no significant difference in Vpeak in PIHS group（P〉0.05）,but the GLPS and strain rate decreased significantly（q=4.35,3.97;P〈0.01）. Conclusion Longitudinal systolic function of myocardium of left ventricle was impaired in patients with pregnancy-induced hypertension syndrome.Velocity vector imaging can display early damage of systolic function of left ventricle in normal pregnant women and in those with pregnancy-induced hypertension syndrome.
