Analysis of radiation dose in 59521 CT examinations

摘  要:目的:分析59521例CT检查的辐射剂量数据,建立医院水平的CT诊断剂量参考水平。方法:使用CT辐射剂量计算软件,回顾性分析本院2015年7月-2016年6月共59521例成人的CT检查数据。按照检查部位分别计算CT容积剂量指数(CTDIvol)、体型特异性剂量估计(SSDE)、剂量长度乘积(DLP)和有效剂量(ED)的中位数及四分位数间距(IQR),取CTDIvol和DLP的第3个四分位数(Q3)为诊断剂量参考水平(DRL),其中胸部CT的数据分为低剂量、单期、多期和全部扫描分别计算,腹部CT的数据分为单期、多期和全部检查分别计算。结果:各部位CTDIvol的DRL:头部为45.2mGy;胸部低剂量为1.3mGy,单期扫描为11.5mGy,多期扫描为11.4mGy,总剂量为11.8mGy;腹部单期扫描为20.6mGy,多期扫描为20.2mGy,总剂量为20.6mGy;冠状动脉为12.9mGy。各部位DLP的DRL:头部为569.8mGy·cm;胸部低剂量为42.0mGy·cm,单期扫描为399.0mGy·cm,多期扫描为968.5mGy·cm,总剂量为474.0mGy·cm;腹部单期扫描为958.0mGy·cm,多期扫描为2419.1mGy·cm,总剂量为2045.1mGy·cm;冠状动脉为328.0mGy·cm。结论:对临床日常工作中CT检查辐射剂量数据的汇总分析有助于在医院水平评估CT辐射剂量。Objective:To establish institutional diagnostic reference level of radiation dose based on CT examination data of 59521 patients. Methods:Radiation dose metrics in diagnostic CT examinations of 59521 adults during July 2015 to June 2016 were retrospectively analyzed by software. The median and interquartile range (IQR) of volume CT dose index (CTDIvol), size specific dose estimate (SSDE), dose-length product (DLP), and effective dose (ED) were calculated according to anatomic region. The CTDIvol and DLP at the 3rd quartile (Q3) were set as the dose reference levels (DRLs). Radiation dose metrics in chest CT were calculated for low-dose, single-phase,multiphase, and whole examination. And in abdomen CT those were calculated for single-phase, multiphase, and whole examination. Results:DRLs of CTDlvo~ for all anatomic regions were as follows: 45.2mGy for head;in chest, 1.3mGy for low-dose, 11.5mGy for single-phase, 11.4mGy for multi- phase,11.8mGy for whole examination; in abdomen, 20. 6mGy for single-phase, 20. 2mGy for multiphase, 20. 6mGy for whole examination;and 12.9mGy for coronary artery. DRLs of DLP were as follows:569.8mGy· cm for head,42.0mGy · cm for chest of low-dose, 399.0mGy· cm for chest of single-phase, 968.5mGy · cm for chest of multiphase, 474.0mGy·cm for whole chest examination, 958. 0mGy·cm for abdomen of single-phase, 2419. lmGy·cm for abdomen of multiphase, 2045. lmGy·cm for whole abdomen examination,and 328.0mGy·cm for coronary artery. Conclusion:Summary and analysis of radiation metrics of CT examination in clinical practice can be helpful for institutional evaluation of CT radiation doses.
