摘  要:目的对过氧化物酶体增殖物激活受体γ(PPARγ)基因外显子6与多囊卵巢综合征(PCOS)的关系进行系统评价。方法检索中英文数据库2016年6月30日之前发表的关于PPARγ基因多态性和PCOS关系的文献资料,应用RevMan5.3软件进行系统评价。采用随机效应模型或固定效应模型计算效应指标:OR值和95%CI进行亚组分析和敏感性分析,用Egger's法通过stata12.0软件定量评估发表偏倚。结果 PPARγ基因外显子6 His447His多态性与PCOS无明显关系。按种族进行亚组分析,在黄种人中共显性模型CC vs CT:Z=2.03(P=0.04);共显性模型CC vs TT:Z=2.88(P=0.004);显性模型:Z=2.54(P=0.01);超显性模型:Z=2.50(P=0.01);隐性模型:Z=2.58(P=0.010);等位基因模型:Z=3.00(P=0.003)。在白种人中共显性模型CC vs CT:Z=2.03(P=0.04)。结论黄种人PPARγ基因His447His的多态性与PCOS易感性有关;且等位基因C和T分别为PCOS的保护和易感因素。白种人中PPARγ基因His447His的多态性可能与PCOS有关,且等位基因C可能是PCOS的易感因素。Objective To explore the relationship between polymorphisms in exon 6 of the peroxisome pro-liferator activated receptor gamma (PPART) and polycystic ovary syndrome (PCOS) by systematic review. Methods We searched the studies published until July 2016 in Chinese- English document database which evaluated the associations of PPAR7 polymorphisms with PCOS, and they were analyzed by RevMan5.3 software. Fixed and random effect models were employed and Odds ratios (ORI with corresponding 95 confidence intervals (CI} were used as the indicator of intervention effect. Subgroup analysis and sensitivity analysis was performed. The method of Egger's by stata12.0 software was applied to evaluate the publica- tion bias. Results The results suggested that there was no obvious association between PPAR7 polymor-phism and PCOS. We performed the subgroup analysis of PPAR7 polymorphism with race elements. A- mong the Asian,Z = 2.88(P = 0.004 ) for CC vs CT in codominant model; Z = 2.54 (P = 0.01 ) for CC vs TT in codominant model; Z = 2.54 ( P = 0.0i ) in dominant model; Z = 2.50 ( P = 0.01 ) in overdominance model;Z =.2.58(P = 0. 010) in recessive model;Z= 3.00(P= 0. 003) in allele model. Among the Caucasian,Z = 2.03 (P = 0. 041 for CC vs CT in codominant model. Conclusions Among the Asian, PPAR7 polymorphisms is related to a decreased risk of PCOS. Allele C may be the protective factors of PCOS and allele T may be the risk factors of PCOS. Among the Caucasian, PPARγ, polymorphisms may be related to PCOS and allele C may be the risk factors of PCOS.
