高血压脑出血血肿引流术患者收缩压变异性增大的影响因素

摘　　要：目的:探讨高血压脑出血行立体定向微创穿刺置管引流术患者急性期收缩压变异性增大的相关因素。方法:纳入70例高血压脑出血行立体定向微创穿刺置管血肿引流术患者,根据神经功能缺损程度绘制的ROC曲线得出的临界值,将患者分为血压变异性(BPV)较大组和BPV较小组,比较两组患者不同时间点收缩压、NIHSS评分、血糖及血脂等指标的差异。结果:BPV较大组患者入院时首次收缩压、第1天平均收缩压、术前收缩压、美国国立卫生研究院卒中量表评分(NIHSS评分)、血清总胆固醇、低密度脂蛋白显著高于BPV较小组,差异具有统计学意义(P<0.05)。结论:入院时首次收缩压、第1天平均收缩压、术前收缩压、NIHSS评分、总胆固醇、低密度脂蛋白及性别是高血压脑出血血肿引流术患者收缩压变异性增大独立影响因素。Objective： To investigate the influence factors which cause rising of systolic blood pres- sure （SBP） variability in acute patients with hypertensive cerebral hemorrhage and accepted minimally invasive catheter hematoma drainage. Methods： Seventy patients with hypertensive cerebral hemor- rhage and accepted stereotactic minimally invasive puncture drainage surgery were included in this in- vestigation and divided into two groups： higher blood pressure variation （BPV） group and lower BPV group according to the critical values from ROC curve on the basis of nerve function defect degree. SBP at different time points, NIHSS values, indicators of blood sugar and blood lipid were compared be- tween the 2 groups. Results： The NIHSS score, the first record of SBP and average SBP on the first day of hospitalization, SBP before the operation, serum levels of total cholesterol, and low density lipo- protein in higher BPV group were significantly higher than those in lower BPV group （ P 〈 0.05 ）. Couclusions ： The NIHSS score, the first record of SBP and average SBP on the first day of hospitaliza- tion, SBP before the operation, Serum levels of total cholesterol, low density lipoprotein and gender were independent influence factors for SBP variation increase in patients with hypertensive cerebral hemorrhage and accepted stereotactic minimally invasive puncture drainage surgery.
