便携式清创冲洗器清创效果的实验研究

摘　　要：目的探讨便携式清创冲洗器对白兔切割伤细菌污染模型抗感染作用的影响。方法普通级雄性新西兰兔30只,按随机数字表法分为对照组、0.5 h冲洗组、6 h冲洗组、24 h冲洗组、48 h冲洗组,6只/组。建立兔外伤细菌感染模型,于不同时间点使用便携式清创冲洗器冲洗伤口,分别于不同时间点进行体温、白细胞计数、细菌学定量检测和组织病理学观察。结果0.5 h和6 h冲洗组,白兔体温无明显波动;细菌计数在致伤后24 h、48 h、72 h明显低于对照组,差异有统计学意义(P<0.05);白细胞数量上升幅度小,与对照组比较差异有统计学意义(均P<0.05)。24 h、48 h冲洗组,白兔体温明显升高;细菌计数和白细胞数量在致伤后24 h、48 h、72 h与对照组接近,差异无统计学意义(均P>0.05);组织病理学检查显示,与对照组比较,0.5 h、6 h冲洗组白兔炎症反应轻微、组织修复明显加快。结论便携式清创冲洗器携带方便,操作简单,在创伤早期可有效清除伤道内细菌和污物,预防创伤感染,促进伤口愈合。Obj ective To investigate the anti-infection effect of portable debridement syringes on bacterial infec-tion in the cutting wounds of rabbits.Methods Thirty common male New Zealand rabbits were randomly divided into five groups （0.5 h,6 h,24 h,48 h irrigation group and control group）with six rabbits in each group.The rabbit model of bacterial infection after trauma was established,and the wound was washed by portable debridement syr-inges at different times.Studies about body temperature measurement,white blood cell counts,bacteriological quan-titative detection and histopathology observation were carried out.Results There was no significant change in the body temperature of rabbits in 0.5h,6h irrigation groups.However,compared with the control group,the bacteria counts were significantly lower at 24 h,48 h,72 h after wounded （P〈0.05）.The white blood cell counts increased slightly（P〈0.05）;In 24 h,48 h irrigation groups,the body temperature was increased significantly,but the bacte-ria counts and the white blood cell counts in 24 h,48 h,72 h after wounded were similar to that in the control group （P〉0.05）.Histopathology examination showed that there were mild inflammatory response and accelerated tissue repair in the 0.5 h,6 h irrigation groups compared with the control group.Conclusions Portable debridement syringe is convenient to carry and the operation is simple.It could effectively scavenge bacteria and dirt in the wound,prevent wound infection and promote wound healing for early trauma.
