摘　　要：目的分析丙型肝炎性肝硬化患者血清内脏脂肪素的表达意义。方法收集河北省沧州市中心医院2014年7月-2015年11月117例丙型肝炎性肝硬化患者（肝硬化组，A级41例、B级42例、C级34例）和20例健康对照人员（对照组）为研究对象，测量两组被试人员的血清内脏脂肪素、胰岛素抵抗指数（HOMA-IR）、自介素-6（IL-6）水平。结果肝硬化组及A、B、C三级患者内脏脂肪素水平低于对照组，HOMA-IR和IL-6水平高于对照组，差异有统计学意义（P〈0．05）。A、B、C三级患者的内脏脂肪素水平比较，差异无统计学意义（P〉0．05），HOMA-IR和IL-6水平比较，差异有统计学意义（P〈0．05）。将内脏脂肪素作为应变量，其他生化指标作为自变量，胰岛素r=-0．237、HOMA-IR r=-0．229、IL-6 r=-0．076，内脏脂肪素与胰岛素、HOMA-IR、IL-6无明显相关性（P〉0．05）。结论丙型肝炎性肝硬化患者的内脏脂肪素表达水平较低，内脏脂肪素可作为诊断丙型肝炎性肝硬化的指标，但是不能作为判断肝功能严重程度的指标。Objective To analyze the significance of serum levels of visfatin in patients with hepatitis C cirrhosis. Methods 117 cases of hepatitis C cirrhosis patients （Liver cirrhosis group, A level 41 cases,B level 42 cases and C level 34 cases） and 20 healthy controls （control group） were collected in Central Hospital of Cangzhou City from July 2014 to November 2015. The serum levels of HOMA-IR, IL-6 and visfatin in two groups were measured. Results The levels of visfatin in A, B, C three level and liver cirrhosis group were lower than those in the control group, HOMA-1R and IL-6 levels were higher than those in the control group, the difference was statistically significant （P 〈 0.05）. A, B, C three group comparison, the difference of the visfatin level of the patients was not statistically significant （P 〉 0.05）, HOMA-IR and IL-6 levels were significantly different （P 〈 0.05）. The visfatin was used as the dependent variable, other biochemical parameters as independent variables, insulin r=-0.237, HOMA-IR r=-0.229, IL-6 r=-0.076, visfatin and insulin, IL-6, HOMA-IR had no significant correlation （P 〉 0.05）. Conclusion The expression of visfatin in pa- tients with hepatitis C cirrhosis is lower, and visfatin can be used as an index to diagnose hepatitis C cirrhosis, but it can not be used as an index to judge the severity of liver function.
