糖宁通络胶囊治疗糖尿病足的临床经验

摘　　要：目的探讨特色苗药糖宁通络胶囊治疗糖尿病足的效果。方法选择2015年8月-2016年1月贵州百灵中医糖尿病医院收治的97例糖尿病足患者，根据治疗方法的不同分为常规西医治疗组（34例）和糖宁通络综合治疗组（63例）。常规西医治疗组接受常规西医治疗，包括口服盐酸二甲双胍或注射胰岛素、双氧水和生理盐水冲洗创面，以及抗生素对症治疗。糖宁通络综合治疗组在常规西医治疗的基础上加用糖宁通络胶囊，每次4粒，3次／d。比较两组患者的Wagner分级、平均创面面积和疼痛改善情况，同时测定血清炎症因子等相关指标。结果糖宁通络综合治疗组和常规西医治疗组的临床治愈率分别为61．9％和38．2％。临床总有效率分别为92．1％和73．5％，差异有统计学意义（P〈0．05）。与治疗前比较，两组患者治疗后溃疡面积、溃疡深度显著减小，疼痛评分显著降低，差异均有统计意义（P〈0．05），且糖宁通络综合治疗组溃疡愈合情况明显优于常规西医治疗组（P〈0．05）。治疗后，两组患者白细胞介素6水平均较治疗前显著下降，人成纤细胞生长因子2（FGF-2）水平均较治疗前显著升高，差异均有统计学意义（P〈0．05），且糖宁通络综合治疗组FGF-2水平显著高于常规西医治疗组（P〈0．05）。结论与常规西医治疗比较。糖宁通络胶囊综合治疗可加速创面愈合，改善患者生活质量，值得临床推广应用。Objective To investigate the therapeutic effect of Miao nationality medicine Tangning Tongluo in the treatment of diabetic foot. Methods Ninety-seven cases of patients with diabetic foot admitted to Guizhou Bailing Diabetes Hospital of Traditional Chinese Medicine from August 2015 to January 2016 were selected and divided into conventional western medicine group （34 cases）, Tangning Tongluo comprehensive treatment group （63 cases） according to different treating methods. The conventional western medicine group received conventional western medicine, including oral Metformin Hydrochloride or Insulin Injection, hydrogen peroxide and normal saline for washing the wound, and antibiotics for symptomatic treatment. On basis of conventional western medicine, Tangning Tongluo comprehensive treatment gYoup was added with Tangning Tongluo Capsules, once 4 granules, 3 times a day. The Wagner classification, average wound area, and improvement of pain between the two groups were compared, and the indexes of serum inflammatory factors were detected. Results The clinical cure rate of Tangning Tongluo comprehensive treatment group and conventional western medicine group was 61.9%, 38.2% respectively, the total effective rate of which was 92.1%, 73.5% respectively, the differences between the two groups were statistically significant （P 〈 0.05）. Compared with before treatment, the area of ulcer, depth of ulcer after treatment in the two groups were reduced, and the pain scores were decreased, the differences were statistically significant （P 〈 0.05）, and the healing situation of ulcer in the Tangning Tongluo comprehensive treatment group was significantly better than that of conventional western medicine group （P 〈 0.05）.
