摘  要:目的探讨公兔骶髓运动诱发电位(SMEP)在不同程度骶髓缺血损伤动物模型急性期的变化特点及与骶髓病理损害的相关性。方法30只公兔随机分为对照组(仅分离腰动脉,n=5),实验组5组(结扎不同数量腰动脉,n=5),术后连续记录SMEP2h,分析SMEP波形变化特点以及与脊髓病理损害程度的相关性。结果1根组与对照组和结扎前比较,差异无统计学意义(P〉0.05);结扎2、3、4、5根组SMEP即刻出现潜伏期延长和波幅降低,分别经过(0.2±0.2)、(0.4±0.3)、(0.3±0.2)和(0.4±0.2)min波平稳;波幅与基线波幅百分比分别稳定在(94.5±5.9)%、(51.6±0.6)%、(9.6±11.0)%、(0.0±0.0)%(P〈0.05);2根组、3根组、4根组和5根组与对照组和结扎前潜伏期比较,差异有统计学意义(P〈0.05);SMEP的波幅变化与术后2d脊髓病理损害程度均呈正相关(r=0.939,P〈0.01)。结论公兔SMEP波对骶髓缺血性损伤敏感,波幅变化能够反映骶髓病理损害程度。Objective To explore the changes of sacral spinal motor evoked potentials (SMEP) in male rabbit models with different degrees of sacral spinal cord ischemia in the acute phase and the correla- tion with sacral spinal cord pathological damage. Methods Thirty rabbits were randomly divided into 6 groups : control group ( n = 5 ), just separating lumbar artery; and 5 experimental groups ( n = 5 each), li- gating lumbar arteries ( 1, 2, 3, 4 and 5). The change patterns of SMEP in male rabbits with spinal cord ischemia in the acute phase were sequentially recorded for 2 h, and the correlation with spinal cord patho- logical damage was analyzed. Results Immediatelly after ligating 2 -5 lumbar arteries, the latency for SMEP prolonged and the amplitude decreased. After ( 0. 2 ± 0. 2 ), ( 0.4 ± 0. 3 ), ( 0. 3 ± 0. 2 ) and (0. 4 ± 0. 2) min, the SMEP was stable. The volatility change of SMEP was stabilized at (94. 5 ± 5. 9)% , (51.6 ± 30. 6) %, (9. 6 ± 11.0 ) % and ( 0. 0 ± 0. 0 ) % respectively ( P 〈 0. 05 ). After ligating 2 - 5 lumbar arteries, the latency was significantly prolonged as compared with control group and that before liga- tion (P 〈 0.05). There was a positive correlation between the volatility change of SMEP and the spinal cord pathological damage after ligation for 2 days ( r = 0. 939, P 〈 0. 01 ). Conclusion The SMEP wave is sensitive to the ischemic injury of sacral cord. The changes of SMEP amplitude can reflect the degree of pathological damage of the sacral cord.
