广西北部湾经济区参加新型农村合作医疗农村居民新农合同城化的意愿调查分析

摘　　要：目的了解参加新型农村合作医疗(新农合)的农村居民对北部湾经济区新农合同城化的意愿,为制定完善新农合政策,提升新农合统筹层次,促进北部湾经济区同城化提供参考依据。方法采用多阶段随机抽样方法抽取在南宁、钦州、北海、防城港四市辖区的乡镇卫生院,对其参加新农合的农村居民进行新农合制度的认知及评价、新农合同城化态度等调查。采用logistic回归模型分析新农合同城化态度的影响因素。结果共调查656人,男性325人,女性331人,年龄(43.8±17.9)岁。赞成新农合同城化有568人(86.6%),不赞成38人(5.8%),不清楚50人(7.6%)。影响参合农村居民同城化意愿的因素有区域、知晓缴费标准、对缴费标准的态度、对提高缴费的意愿以及对医疗服务水平评价。结论区域经济水平、参合农村居民对新农合政策的了解程度会影响新农合同城化的意愿,城镇化、科学的补偿政策、稳定的筹资机制、基层医疗机构服务能力提升更有利于新农合同城化的推进。Objective To investigate the attitude of rural residents in New Rural Cooperative Medical System （NRCMS） to urban integration of Beibu Gulf Economic Zone, and to proposal NRCMS policy, improve integration level of NRCMS, so as to provide some references for accelerating urban integration of Beibu Gulf Economic Zone. Methods Using multi-stage random sampling method, some township health centers from Nanning, Qinzhou, Beihai and Fangchenggang were enrolled, and an investigation on the general data was performed in the rural residents with NRCMS. The data included rural residents＇ recognition and assessment of NRCMS as well as their attitudes to urban integration of NRCMS. The logistic regression model was used to analyze the risk factors for the attitude of rural residents with NRCMS to urban integration. Results A total of 656 individuals involved in the survey. There were 325 males and 331 females, aged （43.8 ~ 17.9 ）years. As for urban integration of NRCMS, approval in 568 （86.6%） residents, disapproval in 38 （5.8%） residents and unclear in 50 （7.6） residents. The factors influencing the intension of rural residents with NRCMS for urban integration were area, the awareness of charging policy, the attitude to charging policy, the attitude to increasing charging standard and the assessment of medical service level. Conclusion The willingness of NRCMS integration could be influenced by the regional economy level and rural residents＇ awareness of NRCMS. Urbanization, scientific compensation policy, stable financing mechanism, the improving service of primary medical institutions are advantageous to promote NRCMS integration.
