中西药联用治疗糖尿病周围神经病变的临床疗效

摘　　要：目的:观察参芪复方联用前列地尔、α-硫辛酸治疗糖尿病周围神经病变(DPN)的临床疗效,并探讨其作用机理。方法:将DPN患者75例随机分为3组,3组患者均常规控制血糖,对照组给予甲钴胺治疗;西药组给予前列地尔联合α-硫辛酸治疗;中西医结合治疗组在给予前列地尔联合α-硫辛酸治疗的基础上口服中药汤剂参芪复方;治疗14天后记录3组患者治疗前后的临床症状,检测下肢神经传导速度(NCV)、感觉振动阈值(VPT)、血清超敏C反应蛋白、丙二醛(MDA)及超氧化物歧化酶(SOD)水平。结果:中西医结合治疗组及西药组临床疗效优于对照组(P<0.05);中西医结合治疗组NCV增加,与西药组及对照组比较差异有统计学意义(P<0.05);中西医结合治疗组VPT降低,与西药组及对照组比较差异有统计学意义(P<0.05);与西药组及对照组比较,中西医结合治疗组超敏C反应蛋白和MDA水平降低(P<0.05),SOD活性增加(P<0.05)。结论:中药参芪复方联用前列地尔、α-硫辛酸可明显改善DNP患者的临床症状,增加神经传导速度,降低感觉振动阈值,其机制可能是通过提高抗氧化能力,减轻炎症反应而实现。Objective： To observe the clinical effects of ShenQi compound recipe combined with Alprostadil and α -lipoic acid in treating diabetic peripheral nemropathy （DPN） and explore its function mechanism. Methods： 75 patients with DPN were randomly divided into three groups, all three groups were given routine blood glucose, the control group was given methylcobalamin; Western medicine group was given Alprostadil and α-lipoic acid; the treatment group of integrated traditional Chinese medicine given oral administration of ShenQi compound recipe decoction on the treatment of Alprostadil and α-lipoic acid; clinical symptoms, the levels of NCV, VPT, serum HS-CRP, MDA and SOD before and after treatment in three groups were recorded after treating in 14 days. Results： The clinical effects of the treatment group of integrated traditional and Chinese medicine and Western medicine group were better than these of the control group obviously （P〈0.05）; NCV in the treatment group of integrated traditional Chinese and Western medicine increased obviously, there was statistical difference compared with Western medicine group and the control group （P〈0.05）; VPT in the treatment group of integrated traditional and Chinese and Western medicine decreased obviously, there was statistical difference compared with Western medicine group and the control group （P〈0.05）; compared with Western medicine group and the control group, the levels of HS-CRP and MDA decreased obviously （P〈0.05）, the activity of SOD increased obviously （P〈0.05）. Conclusion： ShenQi compound recipe combined with Alprostadil and α-lipoic acid can improve clinical symptoms of DNP patients, promote NCV, lower VPT, its mechanism is promoting antioxidant capacity and relieve inflammation reaction.
