应用三柱分型理论有限切开治疗闭合性Pilon骨折的临床研究

摘　　要：目的 探讨应用三柱理论结合有限切开治疗Pilon骨折的临床疗效。方法 自2012年1月-2015年6月,我院应用三柱分型理论结合有限切开对20例Pilon骨折患者进行治疗。结果 4例出现切口边缘少许坏死结痂,经换药后均愈合,余病例均一期愈合。所有患者获得9-16个月随访,平均（12.1±2.3）个月,随访期内无内置物松动、断裂;末次随访骨折均已骨性愈合,平均愈合时间4-7个月,末次随访依据AO/FAS踝-后足评分量表进行临床疗效测评,优9例,良7例,一般2例,差2例。结论 采应用三柱分型理论有限切开治疗闭合性Pilon骨折临床效果满意。Objective To explore the clinical effect of three-column theory combined with limited incision in the treat- ment of Pilon fractures. Methods A total of 20 patients with Pilon fractures were treated with three-column typing the- ory combined with limited incision from January 2012 to June 2015. Results In 4 cases, a few necrotic scabs were found on the edge of the incision. They were all healed after the dressing change. All other cases were healed in phase I. All cases were followed up for 9-16 months, with an average of（12.1±2.3） months. During the follow-up period, there was no loose or fractures of implants; at the last follow-up, all the fractures were healed, with the average healing time of 4-7 months. The last follow-up visit was based on AO/FAS ankle-hind foot score scale for clinical efficacy eval- uation, with excellent in 9 cases, good in 7 cases, general in 2 cases and poor in 2 cases. Conclusion The clinical re- suits of three-column typing theory combined with limited incision in the treatment of closed Pilon fractures are satis- factory.
