白细胞酯酶检测代替涂片细胞学计数评价痰液标本质量

摘　　要：目的比较白细胞酯酶检测和传统涂片细胞计数评价痰液标本质量,并探讨其临床应用价值。方法 2015年6月—2016年3月湖北民族学院附属民大医院各临床科室共4 571份细菌培养痰液标本,根据临床是否检测白细胞酯酶分为两组。临床未筛检组2 068份痰液标本,在微生物实验室涂片染色细胞计数,并用两种试剂检测白细胞酯酶。临床筛检组2 503份痰液标本,用一种试剂检测白细胞酯酶,结果为阳性者的标本送至检验科,在微生物实验室涂片细胞计数,结果为阴性的重新采集,比较两组标本质量。结果临床未筛检组2 068份痰液标本,涂片细胞计数合格率为31.0%,白细胞酯酶检测阳性率(即合格率)为32.2%,两种方法评价痰液标本质量差异无统计学意义(P>0.05);两种国产白细胞酯酶检测试剂之间差异无统计学意义(P>0.05)。临床筛检组,涂片细胞计数,痰液标本合格率为76%,与临床未筛检组差异有统计学意义(P<0.05)。结论 1白细胞酯酶检测用于评价痰液标本质量,与传统的痰液涂片细胞计数有相同的效果;2白细胞酯酶检测用于评价痰液标本质量,是一种经济、快速、简便易行的方法,适宜在临床推广。Objective To compare Leukocyte Esterase test with conventional smear cytological examination for evaluation of sputum specimens and explore its clinical values. Methods 4571 sputum samples in the Affiliated Hospital of Hubei University for Nationalities（ June 2015 to March 2016） were divided into two groups.The non-screening Group（ 2 068 sputum specimens） was gram-stained at the microbiological laboratory and examined for Leukocyte Esterase by two detection reagents.The screening Group（ 2 503 sputum specimens） was examined for Leukocyte Esterase.The positive specimens were sent to the laboratory.The negative specimens were collected again. The specimens＇ qualities of the two groups were compared.Results The qualified rate of sputum specimens was 31. 0% by conventional smear cytological examination and positive rate was 32.2% by Leukocyte Esterase test in the non-screening Group.There were no statistical differences between the two methods for evaluation of sputum specimens（ P〉0. 05）. The qualified rate of sputum specimens was 76% by conventional smear cytological examination in the screening Group.There were statistically significant differences between the two groups（ P〈0. 05）. Conclusions 1 The Leukocyte Esterase test and conventional smear cytological examination have the same effects for evaluation of sputum specimens.2The Leukocyte Esterase test for evaluation of sputum specimens is simple,economic and effective,so it is worth to be widely applied.
