摘 要:目的探讨16排螺旋CT血管造影扫描方向对图像质量影响,提高头颈部多层螺旋CT血管成像(CTA)检查成功率以及重建血管的图像质量,增加病灶检出率。方法选择需要进行头颈部动脉造影的60例患者,将其随机分为观察组和对照组,每组各30例。观察组采用减影法由头侧往足侧扫描,对照组采用减影法由足侧往头侧扫描。原始数据采用容积再现、最大密度投影及多平面重组技术进行后处理,观察各部位血管CT值及血管显像质量,对比2组患者头颈部动脉血管显像质量,并采用3级评分法评估图像质量。结果观察组双侧锁骨下动脉、颈总动脉起始部、头臂干血管显像评分优于对照组(P<0.05)。结论在同等造影剂和辐射剂量下,通过从头侧往足侧静脉团注造影剂扫描,可取得较好血管显像质量,还能得到更多显像质量较好的血管信息,有利于及早发现相关部位的病变。Objective To discuss the influence of scanning direction on image quality in 16-slice spiral CT angiography(CTA),and to increase the success rate of head and neck CTA,image quality of reconstructed vessels and detection rate of lesions.Methods Sixty patients were randomly assigned to received head and neck CTA scan either from head side to foot side(observation group,n=30)or from foot side to head side(control group,n=30).Volumetric reconstruction(VR),maximum intensity projection(MIP)and multi-plane reconstruction(MPR)were used for postprocessing.Vascular CT value and imaging quality were observed in both groups.Furthermore,the image quality was evaluated by using a 3-grade scoring system.Results The scores of imaging quality of bilateral subclavian arteries,initial common carotid artery and brachiocephalic trunk in observation group were better than those in control group(P〈0.05).Conclusion With the same contrast media and equivalent radiation dose,CTA scan from head side to foot side can achieve better vascular imaging quality and provide more vascular information.Therefore,it is helpful to find vascular abnormalities early.
