纯肝素封管频次对维持性血液透析患者临时性中心静脉导管置管的影响

摘　　要：目的探讨纯肝素封管频次对维持性血液透析患者临时性中心静脉导管置管的影响。方法对40例维持性血液透析患者均行临时性中心静脉导管置管及血液透析治疗,并采用纯肝素（肝素钠注射液）封管液对临时性中心静脉导管进行封管。观察40例患者封管频次间隔2、3、4、5、7d血流量及导管通畅率、导管半通畅率、导管栓塞率。结果 40例患者的封管频次间隔2-5d血流量较封管频次间隔7d多,导管通畅率明显高于封管频次间隔7d,导管半通畅率、导管栓塞率均明显低于封管频次间隔7d（均P〈0.05）。结论对维持性血液透析患者临时性中心静脉导管置管,纯肝素封管频次间隔2-5d时,既可延长临时性中心静脉导管的使用寿命,提高血液透析的充分性,又减少封管的次数。Objective To explore the impacts of pure heparin locking frequency on temporary central venous catheterization in maintenance hemodialysis patients.Methods Forty maintenance hemodialysis patients underwent temporary central venous catheterization and hemodialysis treatment.All catheters were locked with pure heparin（heparin sodium injection）.The blood flow volume in catheters and the smoothness,semi-smoothness and occlusion rates of catheters were observed with the locking frequency interval of 2,3,4,5and 7days.Results Compared with the interval of 7days,locking frequency interval of 2-5days resulted in an increase in volume of blood flow and smoothness rate of catheters increased but a decrease in semi-smoothness and occlusion rates of catheters（P〈0.05）.Conclusion Pure heparin locking with a frequency interval of 2-5days can prolong the service life of temporary central venous catheters,improve the adequacy of hemodialysis and decrease the times of catheter locking in maintenance hemodialysis patients.
