长效胰岛素治疗2型糖尿病患者的疗效观察

摘　　要：目的:观察门诊2型糖尿病(T2DM)患者使用长效胰岛素治疗的效果。方法:选取门诊长期随诊的210例T2DM患者,根据随机数字表法,分为对照组(101例)和观察组(109例)。观察组患者给予一日一次长效胰岛素联合口服降糖药治疗,或者一日两次预混胰岛素联合口服降糖药治疗;对照组患者给予单纯使用口服降糖药治疗。3个月后,观察两组患者的空腹血糖水平、餐后血糖水平和糖化血红蛋白水平,以及对两组患者空腹及餐后2 h-C肽水平变化进行对比。结果:经过3个月的治疗后,观察组患者无论是空腹血糖的水平、餐后血糖水平和糖化血红蛋白水平均明显低于对照组,差异有统计学意义(P<0.05)。且治疗后,观察组患者的空腹及餐后2 h-C肽升高值均明显高于对照组,差异有统计学意义(P<0.05)。结论:长效胰岛素联合口服降糖药能有效控制T2DM患者的血糖水平,并降低其糖化血红蛋白水平。Objective： To investigate the effects of long-acting insulin in treatment of outpatients with type 2 diabetes mellitus（ T2DM）. Methods： 210 T2 DM cases,who were long-term followed up,were selected and divided into observation group（ n= 109）and control group（ n = 10） according to the random number table method. The observation group was treated with the long-acting insulin once a day combined with oral hypoglycemic drugs or premixed insulin twice a day combined with the oral hypoglycemic drugs,while the control group was treated with the oral hypoglycemic drugs. 3 months after the treatment,the levels of fasting plasma glucose（ FPG）,postprandial blood glucose（ PPG） and the glycated hemoglobin of the two groups were observed,and the changes of the fasting and 2 h postprandial C-peptide levels were compared. Results： 3 months after the treatment,the levels of FPG,PPG and the glycated hemoglobin of observation group were significantly lower than those of control group（ P〈0.05）. In addition,the increased values of the fasting and 2 h postprandial C-peptide levels of observation group were markedly higher than those of control group（ P〈0.05）.Conclusions： The long-acting insulin could effectively control the blood glucose levels of the T2 DM patients,and reduce the glycated hemoglobin levels.
