摘 要:目的分析手足口病患儿并神经系统损害的磁共振成像(MRI)诊断价值。方法对2015年1月至2016年3月济南军区总医院确诊为手足口病并神经系统损害31例患儿的临床资料进行回顾分析,总结其神经系统损害的MRI表现特征。结果 31例患儿中合并脑干脑炎10例(32.3%),脊髓炎15例(58.1%),脑脊髓炎6例(9.6%)。合并脑干脑炎矢状面T2信号呈斑点状或调片状,轴位病变处呈稍长T1、长T2信号且呈对称性圆点状,双侧脑桥背侧、丘脑、中脑中部呈稍长T1、长T2片状信号;FLAIR系列呈等信号或稍高信号,DWI呈等信号或高信号。合并脊髓炎矢状面病变处呈等T1、长T2信号或长T1、长T2信号,信号呈间断条形或条形,轴位呈等T1、长T2信号或长T1、长T2信号,信号呈圆点状。结论 :手足口病合并神经系统损害在MRI检查中具特异的信号特征,清晰显示受损部位,可为临床诊断提供可靠的依据。Objective To analyze the diagnostic value of magnetic resonance imaging( MRI) in children with hand,foot and mouth disease and nervous system damage. Methods The clinical data of 31 children diagnosed for hand,foot and mouth disease and nervous system damage in the General Hospital of Jinan Military Command from January 2015 to March 2016 were analyzed retrospectively. The MRI performance characteristics of nervous system damage were summarized. Results There were brainstem encephalitis in 10 cases( 32. 3%),myelitis in 15 cases( 58. 1%) and encephalomyelitis in 6 cases( 9. 6%).The MRI of brainstem encephalitis showed that sagittal T2 signal was punctate or patchy,axial lesions showed slightly long T1 and long T2 signal and symmetrical dot shape,bilateral dorsal part of pons,thalamus,midbrain central was slightly long T1,flaky and long T2 signal; FLAIR appeared as an equal signal or slightly high signal and DWI showed equal signal or high signal. The sagittal plane lesions of myelitis were equal T1,long T2 signal or long T1,long T2 signal,the signal was interrupted strip or strip,the axis was equal T1,long T2 signal or long T1,long T2 signal,the signal was round. Conclusion Hand,foot and mouth disease with nervous system damage has specific signal characteristics in the MRI examination. MRI can clearly display the damaged parts,and can provide a reliable basis for clinical diagnosis.
