摘 要:目的比较近视屈光参差患者主导眼与非主导眼的等效球镜度,散光度及眼轴长度,研究主导眼与屈光不正度的关系。方法临床病例回顾性研究。对2009年7月至2014年6月在天津职业大学眼视光工程学院近视屈光参差的学生,经眼部检查排除各种眼病,进行主客观验光、主导眼(卡洞法)及眼轴长度的测定。统计处理采用χ2检验,Wilcoxon符号秩检验及配对t检验。结果主导眼的等效球镜度(-3.65±2.24)D比非主导眼(-2.64±2.20)D高(t=-3.05,P=0.003〈0.005),散光度低(Wilcoxon符号秩检验z=-1.26,P=0.21〉0.05),眼轴更长,分别是(24.63±0.90)mm,(24.22±1.03)mm(t=3.49,P=0.001〈0.005)。尤其是当近视屈光参差介于1.00-2.49D之间时,主导眼散光度数低于非主导眼(Wilcoxon符号秩检验,Z=2.84,P=0.005)。在69例近视屈光参差学生中,右眼为主导眼的占73.9%,左眼为主导眼占26.1%;近视屈光参差在1.00-2.49D和2.50D以上时高度眼与主导眼一致率的分别为76.5%和62.9%。结论近视屈光参差患者主导眼比非主导眼的近视程度深,眼轴长,主导眼倾向于右眼并倾向于与高度眼一致。Objective To investigate the refractive error and axial length between dominant eye and nondominant eye in patients with myopic anisometropia, explore the association between domi- nant eye and refractive error. Methods A total of 69 myopic anisometropia subjects with corrected distance vision acuity ≥0.8 were enrolled, all kinds of eye diseases were excluded. Refractive error, axial length and ocular dominance were measured in all patients. Statistic analysis used χ2 test, Wil- coxon test and Paired t test. Results Dominant eye (-3.65±2.24D) were more myopic than nondomi- nant eye (2.64±2.20D, paired t test, P 〈0.005); less astigmatism (Wilcoxon test, P 〉0.05); dominant eye had a longer axial length than nondominant eye (24.63±0.90mm vs 24.22±1.03mm, respectively, Paired t test, P 〈0.005, Especially when the degree of myopic anisometropia was between 1.00D and 2.49D, dominant eye had lower astigmatism than nondominant eye (Wilcoxon test, P 〈0.05). Right and left ocular dominance was noted in 73.9% and 26.1% of the individuals; dominant eyes were more myopic in 76.5% and 62.9% when the myopic anisometropia was 1.00-2.49D and over 2.50D. Conclusions The present study shows the dominant eye has a greater degree of myopia, ax- ial length than the nondominant eye in myopic anisometropia. The dominant eye tends to be the more myopic eye.
