辅助生殖技术并发急腹症与胚胎移植日血清性激素的关系

摘　　要：目的:分析辅助生殖技术(ART)相关急腹症的临床特点以及与胚胎移植日血清性激素水平的关系。方法:回顾分析2009年1月-2013年12月间,5 020个体外受精-胚胎移植(IVF-ET)助孕周期中急腹症的发生率及其临床特点,化学发光法检测血清雌二醇、孕酮、促黄体生成素、卵泡刺激素水平。分析急腹症的发生与胚胎移植日血清性激素水平的关系。结果:5 020个IVF助孕周期中女方急腹症发生率为3.24%(163例),急腹症组的获卵数和早期流产率明显高于对照组(P<0.05),两组患者平均年龄、移植胚胎数、妊娠率均没有明显差异(P>0.05)。急腹症组胚胎移植日血清雌二醇水平明显高于对照组(P<0.05),但孕酮、促黄体生成素及卵泡刺激素水平没有明显差异(P>0.05)。结论:辅助生殖技术并发急腹症可能导致早期流产率增高,胚胎移植日血清高水平的雌激素是导致辅助生殖技术并发急腹症的高危因素之一。Objective：To investigate the clinical characters of acute abdomen complicated by assisted reproductive technology（ART）,and evaluate its correlation with the sex hormone level on embryo transfer（ET）day.Methods：We retrospectively reviewed 5 020 in vitro fertilization（IVF-ET）cycles of ART between year 2009 and 2013.The levels of E2,P,LH,and FSH were measured by chemi-luminescence method.We analyzed the relation between acute abdomen and the sex hormone level on ET day.Results：The total occurrence rate of acute abdomen after 5 020 ART cycles was 3.24%（163/5 020）.The number of oocytes and the miscarriage rate in acute abdomen group were significantly higher than those in control group（P〈0.05）.There was no significant difference between the two groups in the average age,the average number of transferred embryo and pregnancy rate.The E2 level in acute abdomen group was significantly higher than the control group on the ET day（P〈0.05）,but there was no significant difference among P,LH and FSH.Conclusion：The acute abdomen complicated by ART may cause higher miscarriage rates,and thehigh E2 level on ET day may be one of the risk factors of the acute abdomen.
