摘  要:目的探讨单次与反复使用永久动脉瘤夹对血管壁损伤的影响。方法选用清洁级健康雄性日本大耳白兔30只,采用随机数字表方法将其分为动脉瘤夹首次使用组及反复使用组,每组15只。两组均夹闭颈总动脉30 min,分别取各组颈总动脉标本15段进行血管壁组织病理学损伤的比较。结果首次使用和反复使用组所致的血管壁损伤均可见中层弹力板形变和内皮细胞脱落,反复使用组血管壁出现局部坏死伴发炎性反应。首次使用组及反复使用组血管壁总体损伤程度(U=87.00)和重度损伤率[6.7%(1/15)比26.7%(4/15)]的差异均无统计学意义(均P>0.05)。结论首次使用与反复使用的永久动脉瘤夹均可引起血管壁急性损伤,动脉瘤夹反复使用可致血管壁局部坏死,术中应注意防范。Objective To investigate the effect of different permanent aneurysm clips on common carotid artery wall injury. Methods Thirty clean-grade healthy male Japanese white rabbits were selected.They were divided into a first aneurysm clip use group and a repeated use group using a random number table method( n = 15 in each group). The common carotid arteries of rabbits in both groups were clipped for30 min,and 15 segments of common carotid artery specimens in each group were collected for comparison of pathological injury of vascular wall. Results The clip of first use group and the repeated use group caused vascular wall injury could observe the middle elastic plate deformation and endothelial denudation.The vascular wall had local necrosis with local inflammatory response in the repeated use group. There were no significant differences in the overall damage degree( U = 87. 00) and severe injury rate( 6. 7% [1 /15]vs. 26. 7% [4 /15]) between the 2 groups( all P 〉0. 05). Conclusion The permanent aneurysm clips of the first use and repeated use may cause acute injury of vascular wall. The repeated use of aneurysm clips may result in local necrosis of vessel wall. Precautions should be paid attention to during the operation.
