经皮带线铆钉治疗急性闭合性跟腱断裂23例

摘　　要：背景：急性闭合性跟腱断裂治疗方法较多,带线铆钉在韧带修复及重建方面应用广泛,近年来也被用于跟腱断裂的治疗。目的：探讨：Revo带线铆钉经皮微创治疗急性闭合性跟腱断裂的临床疗效。方法：2011年8月至2014年8月收治23例急性闭合跟腱断裂患者,男15例,女8例;年龄19-46岁,平均（33.6±0.3）岁;左侧13例,右侧10例;均系运动损伤或生活意外伤。MRI检查示跟腱完全断裂,受伤距手术时间1-3 d,采用Revo带线锚钉经皮缝合跟腱断端,末次随访时足踝关节功能依据Arner Lindholm疗效评定标准进行评定。结果：手术顺利,术中未发生重要血管、神经等组织结构的损伤,所有患者均得到12-24个月,平均（15.3±0.3）个月随访,未发生跟腱再断裂及铆钉拔出,腱周无磨损及疼痛等发生,按Arner Lindholm评分标准,优19例,良4例,优良率100%。结论：经皮带线铆钉治疗急性闭合性跟腱断裂效果良好,可降低跟腱再断裂、术后切口感染、皮肤坏死等并发症的发生率,是治疗急性闭合性跟腱断裂的理想方法。Background：There are many methods for the treatment of acute closed rupture of Achilles tendon.Suture anchors are widely used in ligament repair and reconstruction and used in the treatment of Achilles tendon rupture in recent years.Objective：To investigate the clinical effects of percutaneous minimally invasive treatment for acute closed Achilles tendon ruptures with the Revo suture anchors.Methods：Between August 2011 and August 2014,23 patients with acute closed Achilles tendon ruptures were treated with the Revo suture anchors in our department.There were 15 males and 8 females with an average age of（33.6±0.3） years（ranged from 19 to 46 years old）.The left was involved in 13 cases and the right in 10 cases.The rupture was caused by sports injuries or life accidents.MRI showed complete ruptures of the tendon.The interval between injury and operation ranged from 1 to 3 days.Arner Lindholm system was used to evaluate the functional recovery of ankle joints at the last follow-up.Results：The process of the operation was smooth going without the injury of major vessels and nerves.All patients were followed up for 12-24 months（[15.3±0.3]months on average）.No re-rupture,anchor removal,abrasion or pain around the tendons occurred.According to Arner Lindholm system the treatment outcome was excellent in 19 cases and good in 4,with excellence rate of 100%.Conclusions：Percutaneous minimally invasive treatment can reduce the incidence of re-ruptures,infection and skin necrosis and is a good method for acute closed rupture of Achilles tendon.
