重症监护病房铜绿假单胞菌感染影响因素探究及耐药性分析

摘　　要：目的：探究重症监护病房铜绿假单胞菌感染的影响因素，并对其耐药性进行分析。方法：随机抽取2013年5月-2015年9月在笔者所在医院治疗的重症监护病房铜绿假单胞菌感染患者90例，观察患者感染的影响因素，并分析铜绿假单胞菌对药物的耐药性。结果：经对患者进行密切观察可发现对于ICU患者感染铜绿假单胞菌的影响因素主要为机械通气（OR=3．365，95％CI1．408～8．043）、慢性阻塞性肺疾病（COPD）（OR=5．728，95％CI2．643-12．424）、深静脉置管（OR=3．591，95％CI1．666～7．739）；对于药物的耐药性研究表明：铜绿假单胞菌对于常用抗菌药物具有不同的耐药性表现，其对喹诺酮类药物耐药性最低，对头孢类及青霉素类药物的耐药性最高，而对于氨基酸苷类等其他抗菌药物的耐药性较适中。结论：铜绿假单胞菌感染对于重症监护病房内的患者为好发并发症，但其对于常见抗菌药物具有多重耐药性，医护人员应根据具体耐药情况选择相应的抗菌药物进行治疗，以便取得理想的临床治疗效果。Objective： To explore the influencing factors of ICU pseudomonas aeruginosa infection, and to analyze the drug resistance.Method： 90 cases of patients with Pseudomonas aeruginosa infection came form intensive care in the author＇s hospital were Randomly chosen, the factors of patients with infection were observed and the Drug resistance of Pseudomonas aeruginosa were analysised.Result： The patients were observed closely can be found for ICU patients infected with Pseudomonas aeruginosa factors mainly was mechanical ventilation （OR=3.365, 95%CI 1.408-8.043） and chronic obstructive pulmonary disease （COPD） （OR=5.728, 95%CI 2.643-12.424）, deep vein set tube （OR=3.591, 95%CI 1.666-7.739）.For the study of drug resistance showed that Pseudomonas aeruginosa to commonly used antimicrobial agents with different resistance performance, the quinolone resistance of the lowest of cephalosporins and penicillins drug resistance was the highest, and the amino acid glycosides and other antimicrohial drug resistance were moderate.Conclusion： Pseudomonas aeruginosabacteria infection for ICU patients occurred complications, but the to common antimicrobial agents with muhiple Heavy resistance, health care workers should be based on specific drug resistance to select thc appropriate antimicrobial ageuts for treatment, iu order to achieve the desired results of clinical treatmant.
