子痫前期患者血清中PDGF-BB和U-Ⅱ检测的临床价值

摘　　要：目的：研究子痫前期患者血清中血小板衍生生长因子 BB（PDGF-BB）、尿紧张素Ⅱ（U-Ⅱ）水平的变化及临床价值。方法选取赤峰市医院2013年1月～2015年6月产科入院的子痫前期轻度患者52例、子痫前期重度患者36例，另选择40例年龄和孕周匹配的正常孕妇作为对照。通过 ELISA法检测血清PDGF-BB和 U-Ⅱ的水平，绘制 ROC曲线分析确定两种细胞因子诊断子痫前期的临界值，并评价其特异度、敏感度及诊断效能。结果子痫前期患者中，轻度组和重度组的血清PDGF-BB和 U-Ⅱ水平均高于对照组，差异具有统计学意义（t＝5.836～11.841，均P＜0.05），且重度组血清PDGF-BB和 U-Ⅱ水平均明显高于轻度组，差异具有统计学意义（t＝7.133和4.835，均P＜0.05）。血清 PDGF-BB和 U-Ⅱ诊断子痫前期的 ROC曲线下面积分别为0.921，0.927，临界值分别为81.43 ng／L和3.67 ng／L，敏感度分别为86.5％和90.6％，特异度分别为91.4％和87.6％；PDGF-BB和 U-Ⅱ联合检测的诊断敏感度和特异度分别为91.3％和92.2％。结论血清PDGF-BB和 U-Ⅱ水平检测有助于子痫前期的临床诊断，两者联合检测可提高其敏感度和特异度。Objective To study the changes and clinical value of serum platelet derived growth factor BB （PDGF-BB）and uro-tensin Ⅱ （U-Ⅱ）levels in patients with preeclampsia.Methods The cases of obstetric patients suffering from preeclampsia were enrolled in Chifeng Municipal Hospital from January 2013 to June 2015,including 52 cases of mild preeclampsia and 36 cases of severe preeclampsia.40 cases of normal pregnant women with similar age and gestational age were selected as con-trol group.Enzyme-linked immunosorbent assay （ELISA）was adopted to detect the serum level of PDGF-BB and U-Ⅱ.The receiver operating characteristic （ROC）curves were used to detect the cut-off value and analyze the specificity,sensitivity and diagnostic efficiency.Results The serum levels of PDGF-BB andU-Ⅱ were significantly higher in severe preeclampsia group and mild preecla mpsia group than that in control group （t=5.836~11.841,all P〈0.05）.The serum concentration of PDGF-BB and U-Ⅱ were also significantly higher in severe preeclampsia group than that in mild preeclampsia group （t=7.133,4.835,both P〈0.05）.ROC showed that area under the curve of PDGF-BB and U-Ⅱ were 0.921 and 0.927,the cut-off value were 81.43 ng/L and 3.67 ng/L,the sensitivity for PDGF-BB and U-Ⅱ in diagnosis of preeclampsia were 86.5%and 90.6%,the specificity were 91.4% and 87.6%,respectively.The sensitivity and specificity for PDGF-BB and U-Ⅱjoint detection in diagnosis of preeclampsia were 91.3% and 92.2%.Conclusion Detection of serum PDGF-BB and U-Ⅱ level is helpful in the diagnosis of preeclampsia disease.The joint detection of PDGF-BB and U-Ⅱ elevated the sensitivity and speci-ficity in diagnosis of preeclampsia.
