摘  要:目的 :探讨CT肺血管造影(CT pulmonary angiography,CTPA)和碘基物质图联合应用在肺动脉栓塞(pulmonary embolism,PE)诊断中的价值。方法 :选择126例临床怀疑PE患者行肺动脉能谱CT扫描。研究分为3组:单独CTPA组、单独碘基物质图组和CTPA+碘基物质图联合组。比较3组的诊断结果。结果:126例患者中单独CTPA组诊断PE59例,单独碘基物质图组诊断PE 53例,CTPA+碘基物质图联合组诊断PE 75例。3种方法诊断PE病例数有统计学差异(P<0.05),且单独CTPA、单独碘基物质图均分别与CTPA+碘基物质图联合组诊断PE病例数有统计学差异(P<0.05)。结论:CTPA+碘基物质图联合应用诊断PE效能高于单独CTPA及单独碘基物质图,应该成为PE首选影像诊断方法。Objective To investigate the value of combined CT pulmonary angiography (CTPA) and iodine-based material map in the diagnosis of pulmonary embolism (PE). Methods Totally 126 suspected PE subjects underwent pulmonary artery energy spectrum CT examinations, and divided into three groups including a group with CTPA examination (CTPA group ), a group with examination by iodine-based material map (iodine group) and a group with examination by combined CTPA and iodine- based material map (combination group).Results There were 59 PE patients found in CTPA group, 53 cases in iodine group and 75 ones in combination group, and the differences between the numbers of the cases found in the three groups were significant (P〈0.05). The difference between CTPA group and combination group and that between iodine group and combination group were both obvious (P〈0.05). Conclusion Combined CTPA with iodine-based material map behaves better than each of them when used to diagnose PE, and should become the preferred imaging method in the diagnosis of pulmonary artery embolism.
