血清可溶性CD40配体与骨质疏松症并发动脉粥样硬化的相关性

摘　　要：目的探讨骨质疏松症患者血清可溶性CD40配体(solube CD40 ligand,s CD40L)与颈部动脉粥样硬化斑块厚度的关系。方法选取2014-2015年在襄阳市中心医院就诊的250例骨质疏松症(osteoporosis,OP)患者。根据超声将OP患者分为动脉粥样硬化形成组(120例)和动脉粥样硬化非形成组(130例)。结果存在动脉粥样斑块与无动脉粥样斑块的OP患者基本资料比较,年龄、高密度脂蛋白胆固醇(high density lipoprotein-cholesterol,HDL-C)、低密度脂蛋白胆固醇(low density lipoprotein-cholesterol,LDLC)、骨密度以及血清s CD40L浓度比较,差异有统计学意义(P<0.05)。血清s CD40L浓度在OP伴动脉粥样硬化患者中与LDL-C、三酰甘油(triglyceride,TG)以及动脉粥样硬化斑块厚度呈正相关(r=0.256,P=0.033;r=0.258,P=0.031;r=0.288,P=0.016),与HDL-C呈负相关(r=-0.411,P=0.000)。Logistic回归分析显示,血清可溶性CD40L与OP患者动脉粥样硬化相关。结论 OP患者中血清可溶性CD40L在OP患者与动脉粥样硬化厚度呈正相关。在临床上,血清s CD40L检测可能有利于判断OP患者动脉粥样硬化斑块的厚度。Objective To investigate the relationship between serum soluble CD40 ligand （sCD40L） and the risk of atherosclerosis in patients with osteoporosis （ OP）. Methods A total of 250 OP patients were included from 2014 to 2015. All OP patients were divided into atherosclerotic groups （n = 120） and non-atherosclerosis groups （n = 130） ac- cording to ultrasonic examine. Results In OP patients with atherosclerosis, age, high density lipoprotein cholesterol （ HDL-C ） , low density lipoprotein cholesterol （ LDL-C ） , bone mineral density and serum sCD40 liqand had statistical differences compared to those in the group without atherosclerosis （ P 〈 0. 05 ）. Serum sCD40 liqand concentration in OP patients with atherosclerosis was positively correlated with LDL-C, triglycerides （TG） , and atheroselerotic plaque thick- ness （r = 0. 256, P = 0. 033 ; r = 0. 258, P = 0. 031 ; r = 0. 288, P = 0. 016）, and negatively correlated with HDL-C （r = 0.41i, P = 0. 001 ）. In Logistic regression analysis, soluble CD40 liqand was associated with OP patients with ath- erosclerosis. Conclusion Serum soluble CD40 liqand positively was Correlated with atherosclerotlc plaque thickness. Clinically, serum soluble CD40 liqand may help to estimate atherosclerotic plaque thickness in patients with OP.
