芪苈强心胶囊治疗80岁以上高龄多病因慢性心力衰竭患者疗效及安全性研究

摘　　要：目的:观察芪苈强心胶囊治疗80岁以上高龄多病因慢性心力衰竭患者的疗效及安全性。方法:选取我院2012—2014年入院的80岁以上高龄多病因慢性心力衰竭患者共87例,随机分为治疗组和对照组,对照组常规给予限制液体入量、ACEI或ARB、螺内脂、β-受体阻滞剂等抗心衰治疗;治疗组在此基础上给予芪苈强心胶囊口服,4粒/次,3次/d,观察两组治疗前及治疗后14 d、4周的NYHA分级、左心射血分数(LVEF)、血浆N末端B型利钠肽前体(NT-pro BNP),芪苈强心胶囊相关不良反应发生率并监测肝肾功能、血糖等指标。结果:治疗14 d后两组各指标均有显著改善,总有效率、血浆NT-pro BNP下降幅度无明显区别;治疗4周后治疗组总有效率及显效率明显高于对照组(P<0.05),血浆NT-pro BNP下降幅度及LVEF改善幅度明显高于对照组(P<0.05);治疗组未出现芪苈强心胶囊相关不良反应,耐受性好。结论:常规抗心衰治疗基础上加用芪苈强心胶囊长期服用能够更好改善≧80岁高龄多病因心力衰竭患者症状,安全有效。Objective： To observe the effects and safety of Qiliqiangxin capsule on over 80 years old patients with multifactor heart failure. Methods： 87 cases of over 80 years old patients with multifactor heart failure treated in our hospital from 2012 to 2014 were selected and divided into the control group （received routine drug treatment） and the therapy group （received Qiliqiangxin capsule, 4 capsules each time, 3 times a day, based on the routine treatment）, treatment lasted 4 weeks. NYHA cardiac fimctional grades, LVEF, changes of plasma concentrations of NT-proBNP and adverse drug reaction of two groups before treatment, 14 d and 4 weeks after treatment were observed and indexes such as liver and kidney function, blood sugar were monitored. Results： The clinical symptoms of both groups were relieved significantly 14 days after treatment, the difference of the total effective rate and plasma concentrations of NT-proBNP was of no statistical difference; 4 weeks after treatment, the total effective rate of the therapy group was significantly higher than that of the control group （P 〈 0.05）, the NT-proBNP and LVEF concentrations of the therapy group decreased significantly compared with that of the control group （P 〈 0.05）. No adverse events were associated with Qiliqiangxin capsule. Conclusion： Qiliqiangxin capsule could significantly relieve the clinical symptoms of over 80 years old patients with multifactor heart failure based on routine treatment. It was safe and reliable.
