中药综合治疗输卵管妊娠腹腔镜剖管取胚术后的疗效观察

摘　　要：目的:研究重楼血竭散联合盆复康汤治疗输卵管妊娠腹腔镜剖管取胚术后的作用及其对患者血清IL-2、IL-6浓度水平的影响。方法:选取肇庆市中医院妇产科2014年3月—2016年3月肇庆市中医院妇产科收治的输卵管妊娠患者60例,随机分为治疗组和对照组,每组30例。两组均采用输卵管妊娠腹腔镜剖管取胚术,术后常规抗感染治疗3 d。治疗组于月经来潮第7天开始内服重楼血蝎散,配合盆复康汤保留灌肠;对照组仅进行随访观察。观察患者治疗前后的临床表现及体征,采用中医证候评分进行疗效评价;采用子宫输卵管造影术观察治疗后患侧输卵管的通畅情况;比较治疗前后患者血清IL-2、IL-6浓度水平情况。结果:治疗组脱落病例2例,对照组脱落病例1例。治疗组总有效率为89.2%,对照组总有效率为65.5%,提示治疗组总疗效优于对照组(P<0.05)。治疗3个月后,治疗组的患侧输卵管通畅率明显高于对照组,差异存在统计学意义(P<0.05)。血清IL-2、IL-6浓度水平均明显低于对照组,差异存在统计学意义(P<0.05)。结论:重楼血竭散联合盆复康汤能改善输卵管妊娠腹腔镜剖管取胚术后患者的临床症状,提高术后患侧输卵管的通畅率,还可以降低患者术后的血清IL-2、IL-6浓度水平,疗效显著,其作用机制可能通过直接改善局部循环及减少炎症反应从而达到通调管道的作用。Objective： To study the effect of Chonglou Xuejie Powder combined with Peiafukang decoction in the treatment of tubal pregnancy after laparoscopic embryo surgery and its effect on serum IL-2, IL-6 concentration level of patients. Methods： 60 cases of tubal pregnancy patients treated in Zhaoqing Municipal Hospital of traditional Chinese medicine from March 2014 to March 2016 were selected and randomly divided into the treatment group and the control group, with 30 cases in each group. Both groups received laparoscopic tubal pregnancy cesarean section surgery and postoperative routine anti infection treatment for 3 days. The treatment group received Chonglou Xuejie powder combined with Penfukang decoction retention enema on the seventh day of menstrual period while the control group received only follow-up. The clinical manifestations and signs of patients before and after treatment were observed, TCM syndrome score was used for efficacy evaluation; the patency of fallopian tube of the affected side was observed through hysterosalpingography; serum IL-2, IL-6 concentration level of patients before and after treatment was compared. Results： 2 cases dropped out in the treatment group and 1 case dropped out in the control group. The total effective rate of the treatment group was 89.2% and that of the control group was 65.5%, indicating that the effect of the treatment group was better than that of the control group （P 〈 0.05）. After 3 months of treatment, the tubal patency rate of the affected side of the treatment group was significantly higher than that of the control group, there were statistically significant differences （P 〈 0.05）. Serum IL-2, IL-6 concentrations of the treatment group were significantly lower than that of the control group, there were statistically significant differences （P 〈 0.05）.
