肺心病心衰患者心功能与血尿酸水平的相关性研究

摘　　要：目的探讨肺心病心衰患者心功能与血尿酸水平之间的相关性,为患者病情发展及预后判断提供依据。方法将本院收治的100例肺心病心衰患者作为研究对象,设为观察组,按NYHA心功能分级法分为三组,采用全自动生化仪测定血尿酸水平,采用血气分析仪测定PaO2,采用超声心动图测定LVEF、LVED,同期入组50例健康者作为对照组。结果观察组高尿酸血症发病率为56%,高于对照组的28%,差异具有统计学意义(χ2=10.500,P<0.01),观察组血尿酸水平(465.4±108.3)μmol/L,显著高于对照组(340.6±101.2)μmol/L,差异具有统计学意义(t=5.495,P<0.01)。Ⅳ级组尿酸水平明显高于Ⅱ级组及Ⅲ级组,Ⅲ级组尿酸水平明显高于Ⅱ级组,差异具有统计学意义(均P<0.01)。血尿酸与LVED呈正相关(r=0.678,P<0.01);血尿酸与LVEF呈负相关(r=-0.649,P<0.01)。结论肺心病心衰患者血尿酸水平与心衰程度呈正相关,血尿酸水平可作为评估心衰程度的指标。Objective To analyze the relationship between serum uric acid level and heart function in patients with pulmonary heart disease,and to Provide evidence for judging the development and prognosis of patients with pulmonary heart disease.Methods 100 cases of heart failure in patients with pulmonary heart disease were selected as research object,which were divided into three groups according to NYHA classification of cardiac function,Blood uric acid were measured by full automatic biochemical analyzer,the LVEF,LVED were measured by Echocardiography,the PaO2 were measured by blood gas analyzer,Compare the test results with 50 healthy persons.Results The incidence of hyperuricemia in the observation group was 56%,higher than control group（28%）,and the difference was statistically significant（χ2=10.500,P〈0.01）;Observation group,the level of uric acid is（465.4±108.3）μmol/L,higher than in the control group（340.6±101.2）μmol/L,and the difference is statistically significant（t=5.495,P〈0.01）;serum uric acid levels of IV group were higher than gradeⅡgroup and gradeⅢ group（P〈0.05）;LVED and uric acid was positively correlated（r=0.678,P〈0.01）,and LVEF was negative correlation（r=-0.649,P〈0.01）.Conclusion Serum uric acid level was positively correlated with the degree of heart failure in patients with pulmonary heart disease,and serum uric acid level could be used as an index to assess the degree of heart function.
