摘　　要：目的探讨先天性房间隔缺损(ASD)合并左上腔静脉畸形(LSVC)的手术治疗及疗效。方法回顾性分析40例ASD合并LSVC患者临床资料和手术治疗方法及效果。结果患者术前检查ASD合并LSVC漏诊率15%(6/40)。所有患者进行心房间隔缺损直视修补术。在术中发现合并LSVC。依据LSVC引流情况进行相应术中处理。经手术房间隔缺损修补完好,手术成功率100%。术后未出现并发症。随访1年未出现死亡,出现1例并发症。心胸比率较术前缩小(P<0.05),而心室结构较术前有显著变化(P<0.05)。结论通过手术治疗ASD合并LSVC可显著改善心脏结构和功能,疗效良好,并发症少,术后恢复好。因此手术治疗可以作为合并LSVC的优选治疗方式。Objective To analyze the operation treatment and its effect of congenital atrial septal defect （ASD） complicated with left superior vena cava （LSVC） abnormality. Methods Retrospective analysis of clinical data, operation method and treatment outcome was performed in 40 cases of congenital ASD complicated with LSVC abnormality. Results The rate of missed diagnosis of ASD complicated with LSVC abnormality by preoperative examination was 15% （6/40）. Septal defect was dosed with patch repair as the operation treatment. Concurrent LSVC abnormality was found in all patients during the operation. Treatment for LSVC abnormality was based on its drainage. Septal defect repair was in good condition, with operation success rate of 100%. No postoperative complication was recorded. Patients were followed up for 12 months, and all patients were survive, with only one complication. The cardiothoracic ratio was significantly reduced, and ventricular structure was significantly altered （P 〈0. 05）. Conclusion The operation treatment of ASD complicated with LSVC abnormality can significantly improve the cardiac structure and function with good efficacy, few complications, and good postoperative recovery. Therefore the operation treatment is the preferred way for congenital ASD with LSVC abnormality.
