关节镜下两种手术方法治疗中年单纯ⅡC型上盂唇前后损伤的早期疗效对比

摘　　要：目的:观察关节镜下单纯盂唇修复与肌腱切断及固定两种手术方法治疗单纯ⅡC型上盂唇前后(superior labrum from anterior to posterior,SLAP)损伤早期疗效。方法:回顾性分析关节镜下确诊为单纯ⅡC型SLAP损伤的中年患者22例,且所有患者均为单侧损伤,按照不同的手术方式将其分为两组:设盂唇单纯修复术组为对照组(n=11);肌腱切断及固定组为观察组(n=11)。观察记录两组患者术后6、12个月肩关节的加州大学洛杉矶分校(University of California at Los Angeles,UCLA)评分和美国肩肘外科医师(American Shoulder and Elbow Surgeons Scale,ASES)评分。结果:观察组术后6个月及12个月肩关节UCLA功能评分与ASES评分,均高于对照组,差异有统计学意义(P<0.05)。结论:对于中年单纯ⅡC型SLAP损伤,肌腱固定组术后早期肩关节功能恢复优于单纯盂唇修复组。Objective：To observe the early effect of simple labrum repair and tendon cut and fixed under arthroscopy in the treatment of simplex II C type superior labrum from anterior to posterior （SLAP）lesion. Methods：Retrospectively analyzed 22 cases of middle-aged patients who were diagnosed under arthroscopy with simplex Ⅱ C type SLAP lesion, and all patients were unilateral lesions. According to different surgical methods, the patients were divided into two groups： simple labrum repair group was the control group（n = 11 ） ,tendon cut and fixed group was the Observation group（ n = 11 ）. Observed and recorded the University of California at Los Angeles （UCLA） scores and the American Shoulder and Elbow Surgeons Scale （ASES） scores of the two groups of patients 6 and 12 months after surgery. Results：The UCLA function score and ASES score of the shoulder joint in the observation group were significantly higher than those in the control group at 6 and 12 months after operation （ P 〈 0.05 ）. Conclusion ： For middle-aged simplex type Ⅱ C SLAP lesion,the tendon fixation group is better than simple labrum repair group for early recovery of shoulder function.
