护理质量持续改进在降低压疮风险管理中的体会

摘　　要：目的:研究并探讨护理质量持续改进在我院降低压疮风险管理中的应用效果。方法:自2013年3月开始对入住渠县人民医院各病房符合压疮首次评估筛查条件的患者,入院后2 h内即采用Braden评分量表对患者进行首次压疮风险评估,住院期间根据患者病情、用药变化进行动态评估,对压疮高风险患者实施护理预防措施,加强对压疮高风险患者的监管,护理质量持续改进。将2013年3月至2015年9月申报的难免压疮例数和发生压疮例数进行统计,设置该阶段为观察组,改进前的2010年8月至2013年2月阶段设置为对照组,对其难免压疮申报例数和发生压疮例数进行统计,并与观察组进行对比。结果:对照组共申报205例难免压疮,其中压疮发生25例38处;观察组共申报286例难免压疮,其中压疮发生9例15处。两组压疮发生率进行对比,差异显著(P<0.05)。结论:在难免压疮高风险患者的管理过程中,有效落实预防措施,持续护理质量改进,能够降低压疮风险,减少压疮发生。Objective：To study and explore the application effect of nursing continuous quality improvement in our hospital to reduce the risk of pressure ulcer. Methods：Since March 2013,the first pressure ulcer risk assessment was performed 2 h after patients admitted to hospital with Braden rating scale. During hospitalization,according to the patient＇ s condition,medlcation changes,dynamically assess the high risk of pressure ulcer patients to improve nursing preventive measures to strengthen the supervision of patients with high risk of pressure ulcers. The declared number of inevitable ulcers and the actual incidence of pressure ulcer were recorded. Records from March 2013 to September 2015 were set as observation group and compared with control group from August 2010 to February 2013. Results：In the control group,225 cases were reported, pressure ulcers occurred in 25 cases （38 places） ;there were 276 cases of pressure ulcer in the observation group, pressure ulcers occurred in 9 cases （15 places）. The incidence of pressure ulcer in the two groups was compared, and the difference was significant （ P 〈 0.05 ）. Conclusion ：In the management of patients with high risk of pressure ulcers, effective implementation of preventive measures, continuous improvement of quality of care, can reduce the risk and incidence of pressure ulcer.
