摘 要:目的对个性化肝癌患者肝脏周围的动脉血管进行三维容积重建,并探讨其潜在的临床及科研应用。方法针对个性化肝癌患者,通过GE Lightspeed 64排螺旋CT进行扫描,基于300层腹部连续断层图像,利用Minics进行三维图像的容积重建,获取肝脏周围动脉血管的三维结构,结合临床探讨其在疾病诊疗和科研方面的指导作用。结果利用三维容积重建得到了局部腹腔干、肝动脉、肝固有动脉、胃左动脉等血管的三维容积模型。其中,腹腔干血管直径在12.7~18.6 mm之间,肝动脉血管直径约为5.3 mm、肝固有动脉血管直径约为4.5 mm、胃左动脉血管直径约为4.6 mm。结论利用三维容积重建技术所得的肝脏周围血管模型提供了较二维图像更直观的解剖显示和较好的后处理优势,是个性化有限元模型的基础,为肝动脉血流的有限元分析提供了数学模型,对制定更合理的肝脏外科手术方案及术后评价具有一定的指导意义。Objective To carry out the three-dimensional volume reconstruction of hepatic arteries for an individual hepatic cellular cancer( HCC) patient, and discuss its potentials in clinical and research applications. Methods GE Lightspeed 64 multi-detector helical CT was used for patient examination,from which 300 slices were employed for three-dimensional volume reconstruction by utilizing the commercial software Minics,and for further discussion of its potential applications. Results A three-dimensional model including partial celiac axis( about 12. 7-18. 6 mm),hepatic artery( about 5. 3 mm),proper hepatic artery( about 4. 5mm) and left gastric artery( about 4. 6 mm) were reconstructed via the three-dimensional volume reconstruction. Conclusions The three-dimensional model reconstructed by the volume reconstruction provides a more visual display than the original CT images,and has better post-processing advantages. The reconstructed model is the base of finite element model and provides a better mathematical model for future hemodynamic simulations of hepatic arteries, which is helpful for appropriate liver surgery and post-surgical evaluations.
