摘  要:目的探讨100 k Vp、碘克沙醇注射液270 mg I/ml及自适应迭代重建技术(ASIR)在冠状动脉CT血管成像(CCTA)中的应用价值。方法 100例患者行CCTA检查,体质量指数(BMI)均小于28 kg/m2,扫描参数管电压100 k Vp,管电流500 m As,造影剂碘克沙醇注射液270 mg I/ml,采用前置性门控成像序列及40%权重ASIR。扫描后图像传入工作站进行后处理,图像质量由2名高年资放射科医生进行双盲法评分(5分法)。测量图像主动脉根部(AO)、左主干(LM)、左前降支(LAD)、左回旋支(LCX)及右冠状动脉(RCA)开口处的CT值及噪声值、周围脂肪的CT值及噪声值,计算信噪比(SNR)、对比噪声比(CNR),并记录射线剂量(CTDIvol,DLP,ED)及碘摄入量进行统计。结果所测冠状动脉的CT值均在300 Hu以上,能够达到临床诊断要求,图像质量评分均在3分以上,辐射剂量及碘摄入量低于既往文献报道的常规前门控扫描技术(管电压120 k Vp,管电流500 m As,对比剂碘普罗胺370 mg I/ml,滤波反投影(FBP)重建。结论宝石能谱CT采用管电压100 k Vp、对比剂碘克沙醇注射液(270 mg I/ml)及40%权重ASIR行CCTA检查可以获得良好的图像质量,完全能够满足临床诊断要求,同时降低了患者的辐射剂量及碘摄入量。Objective To investigate the feasibility of using 100 k V, Visipaque 270 mg I/ml and adaptive iterative reconstruction technique in coronary computed tomography angiography(CCTA). Methods One hundred patients(BMI28 kg/m2) underwent CCTA were scanned with the tube voltage of 100 k Vp, tube current of 500 m As, and injected with iso-osmolarity contrast agent Visipaque 270(270 mg I/ml) with perspective cardiac gating and ASIR 40%reconstruction. After scanning the images transmited to the workstation for reprocessing. The images were assessed subjectively by two experienced radiologists with double-blindly. Five ROIs were placed onto the ascending root of aorta(AO), left main artery(LM), left anterior descending(LAD), left circumflex artery(LCX), right coronary artery(RCA), and measured their CT values, noise value, backgound CT values, noise value, and calculated signal noise ra-tio(SNR) and contrast noise ratio(CNR). The radiation dosages(CTDIvol, DLP and ED) and iodine intake were also recorded to statistics. Results The results of coronary CT values were above 300 Hu, it could meet clinical diagnostic requirement, image quality scores were above three points, radiation dose and iodine intake were lower than the previous literature reports of conventional scanning with perspective cardiac gating(tube voltage 120 k Vp, tube cur-rent 500 m As, contrast agents iopromide 370 mg I / ml, FBP reconstruction). Conclusion CCTA in gem spectrum CT of using tube voltage 100 k Vp, Visipaque 270 mg I/ml and ASIR 40% iterative reconstruction technique can obtain good image quality, and fully satisfy clinical diagnostic requirement, while reduce the patient′s radiation dose and iodine intake.
