慢性鼻窦炎鼻腔分泌物的细菌类型分析

摘　　要：目的分析慢性鼻窦炎（chronic rhinosinusitis,CRS）伴鼻息肉（chronic rhinosinusitis with nasal polyps,CRSwNP）、不伴鼻息肉（chronic rhinosinusitis without nasal polyps,CRSsNP）以及正常对照组患者的中鼻道的主要细菌的类型。方法选取2014-08~2016-03收治入院的CRSwNP患者（A组）165例,CRSsNP患者（B组）76例,正常对照组（C组）44例。应用鼻内镜引导下用鼻拭子在中鼻道中采取鼻分泌物标本进行细菌培养并进行类型分析。结果三组中最常见的需氧细菌是凝固酶阴性葡萄球菌、棒状杆菌属、金黄色葡萄球菌和流感嗜血杆菌,厌氧细菌在三组中均较少。所有细菌的培养阳性率在三组之间比较差异均无统计学意义（P〉0.05）。结论 CRSwNP、CRSsNP患者和正常对照组中,鼻腔中鼻道的细菌类型没有明显区别。Objective To compare the predominant bacteriologic profiles among middle meatus of chronic rhinosinusitis（ CRS） complicated with nasal polyps（ CRSwNP） and without nasal polyps（ CRSsNP）,and the controls subjects. Methods A total of 165 CRSwNP patients,76 CRSsNP patients,44 control subjects were recruited. Swab samples were obtained from the middle meatus during endoscopic sinus surgery and processed for the presence of aerobic and non-aerobic bacteria culture. Results Mostly aerobic and facultative anaerobic bacteria were detected in the samples from the three groups,in which the most common aerobic bacteria were coagulase-negative Staphylococci,Corynebacterium species,Staphylococcus aureus and Haemophilus influenzae. In contrast,comparatively few anaerobic bacteria were isolated from the three groups. The isolation rates for aerobic and anaerobic bacteria were not significantly different among the three groups. Conclusion The bacteriologic profiles in the nasal middle meatus are not significantly different in Chinese CRSwN P and CRSsN P patients or control subjects.
