摘  要:目的探讨妊娠期糖尿病(GDM)患者血清、脐血、胎盘组织中视黄醇结合蛋白4(RBP4)表达水平及临床意义。方法选取2014年l~12月浙江省嘉兴市妇幼保健院收治的40例GDM孕妇为GDM组和40例糖耐量正常的孕妇为NGT组,用逆转录聚合酶链反应检测胎盘组织中RBP4mRNA的表达,用酶联免疫吸附试验检测孕妇血清RBP4及脐血RBP4水平,计算体重指数,检测空腹血糖、空腹胰岛素、血脂,计算胰岛素抵抗指数(HOMA—IR),并记录新生儿出生体重。结果两组空腹血糖、三酰甘油、HOMA—IR及新生儿出生体重比较,差异有统计学意义(P〈O.05)。GDM组血清、脐血RBP4水平均高于NGT组,差异有统计学意义(P〈0.05)。两组脐血RBP4水平均低于血清RBP4水平,差异有统计学意义(P〈0.05)。GDM组胎盘组织中RBP4mRNA表达水平显著高于NGT组(P〈0.01)。血清RBP4水平与三酰甘油、新生儿出生体重呈正相关(r=0.489、0.488,P〈0.01);脐血RBP4水平与新生儿出生体重呈正相关(r=0.327,P〈0.05)。结论GDM孕妇体内RBP4表达水平与妊娠期糖代谓十异常和胎儿生长发育有关,其可能参与GDM发病。Objective To investigate expression and clinical significance of serum, cord blood and placental tissue retinol-binding protein (RBP) 4 in patients with gestational diabetes mellitus (GDM). Methods 40 cases of GDM in Maternal and Child Health Care Hospital of Jiaxing City, Zhejiang Province from January to December 2014 were se- lected as GDM group, and 40 normal pregnant women with impaired glucose tolerance were selected as NGT group. The expression of RBP4 mRNA in placenta tissue was detected by reverse transcription polymerase chain reaction. Ma- ternal serum and cord blood RBP4 levels were detected by enzyme-linked immunosorbent assay. Body mass index was calculated, fasting blood glucose, fasting insulin, blood lipid levels were detected, HOMA-IR was calculated, birth weight of newborn was recorded. Results Fasting blood glucose, triglyceride, HOMA-IR and birth weight of newborn between two groups were compared, with statistical differences (P 〈 0.05). Serum, cord blood RBP4 levels of GDM group were higher than those of NGT group, with statistical differences (P 〈 0.05). Cord blood RBP4 levels of two groups were lower than serum RBP4 levels, with statistical differences (P 〈 0.05). Placental tissue RBP4 mRNA of GDM group was significantly higher than that of NGT group (P 〈 0.01). Serum RBP4 level was positively correlated with triglyceride, birth weight of newborn (r = 0.489, 0.488, P 〈 0.01). Cord blood level was positively correlated with birth weight of newborn (r = 0.327, P 〈 0.05). Conclusion RBP4 expression levels in GDM women are closely correlated with gesta- tional glycometabolism abnormality, fetal growth and development, it may be involved in the pathogenesis of GDM.
