贵港地区泌尿生殖道支原体感染的检测及耐药性分析

摘　　要：目的了解本地区泌尿生殖道感染和耐药情况,指导临床合理用药。方法使用支原体分离、鉴定及药敏试验盒对13 143例标本进行解脲脲原体(UU)和人型支原体(MH)检测及药敏试验。结果 13 143例患者中支原体阳性3 276例,总感染率24.9%,其中男685例,阳性546例,阳性率14.8%,女458例,阳性730例,阳性率28.9%;其中UU阳性2 355例,MH阳性45例及UU+MH阳性876例。女患者支原体阳性率28.9%明显高于男性14.8%,两者比较差异有统计学意义(P<0.01)。药敏结果:UU、MH、UU合并MH感染对交沙霉素、美满霉素、强力霉素、四环素均为敏感,对诺氟沙星、司帕沙星耐药率最高。结论本地区泌尿生殖道支原体感染以UU为主,交沙霉素等对UU、MH、UU合并MH感染敏感较高,交沙霉素、美满霉素、强力霉素、四环素是治疗本地区泌尿生殖道感染的首选药物。临床用药应根据支原体的种类和药物敏感进行选择。Objective To understand urogenital infection and drug resistance situation in local region for guiding rational medication in clinic.Methods The mycoplasma separating,identification and drug susceptibility test kits were used to perform the detection of Ureaplasma urealyticum（UU）and Mycoplasma hominis（MH）and drug susceptibility test in 13 143 samples.Results Among 13 143 patients,there were 3 276 cases of mycoplasma positive,the overall infection rate was 24.9%;in which 685 cases were male and 546 cases were positive with the positive rate of 14.8%;458cases were female and 730 cases were positive with the positive rate of 28.9%;2 355 cases were UU positive,45 cases were MH positive and 876 cases were UU＋MH positive.The Mycoplasma positive rate in females was significantly higher than that in males,the difference was statistically significant（28.9% vs.14.8%,P〈0.01）.The results of drug susceptibility test：UU,MH,UU combined with MH infections were sensitive to josamycin,minocycline,doxycycline and tetracycline and had the highest resistance rate to norfloxacin and sparfloxacin.Conclusion Mycoplasma infection in local region is dominated by UU,josamycin has higher sensitivity to UU,MH and UU combined with MH infections,josamycin,minocycline,doxycycline and tetracycline are the first choice of drugs for treating urogenital infection in local region.Clinical medication should be selected according to the kinds of mycoplasma and drug susceptibility.
