延期切开复位内固定治疗开放性Pilon骨折疗效观察

摘　　要：目的探讨采用延期切开复位内固定术治疗开放性Pilon骨折的临床疗效。方法 39例开放性Pilon骨折患者,采用Ⅰ期清创缝合,跟骨结节骨牵引术,Ⅱ期切开复位内固定,骨缺损植骨术治疗,术后早期功能锻炼,晚期负重。结果术后按照Baird等踝关节功能评分系统评定:优18例,良13例,可6例,差2例,优良率79.5%。结论采用延期切开复位内固定治疗开放性Pilon骨折对已受伤的软组织损伤小,有利于创面及骨折的一期愈合,并获得良好的踝关节功能。Objective To investigate clinical effect by delayed open reduction internal fixation in the treatment of open Pilon fracture. Methods A total of 39 patients with open Pilon fracture received stage I debridement and suturing, calcaneus and nodules bone traction, stage Ⅱincision fixation internal fixation, and bone defect grafting, followed by early postoperative functional training and late weight training. Results Postoperative Baird ankle joint function scores showed 18 excellent cases, 13 good cases, 6 moderate case and 2 bad cases, with good rate as 79.5%. Conclusion Implement of delayed open reduction internal fixation in the treatment of open Pilon fracture shows little damage on injured soft tissue. This method is beneficial for primary healing in wound and fracture, along with good ankle joint function.
