温经化瘀法治疗原发性痛经(寒凝血瘀证)即时止痛的临床研究

摘　　要：目的以即时止痛为主要疗效指标,探讨温经化瘀法治疗原发性痛经寒凝血瘀证的临床疗效。方法选取90例原发性痛经寒凝血瘀证患者,随机分为中药冲击组、中药常规+穴位按压组、西药组各30例,分别通过香延止痛方配方颗粒冲击给药、香延止痛方配方颗粒常规给药+穴位按压和布洛芬治疗,连续3个月经周期,停药后随访3个月经周期,观察对比临床疗效。结果中药冲击与中药+穴位按压两组均能明显缩短患者腹痛时间、缓解伴随症状,效果优于西药(P<0.05);在改善患者腹痛程度和即时止痛疗效方面与西药相同(P>0.05);停药后随访3个月经周期,中药冲击与中药+穴位按压两组在腹痛疗效、中医证候积分方面,疗效均优于西药组(P<0.01)。结论香延止痛方中药冲击法与中药+穴位按压法对原发性痛经寒凝血瘀证有显著疗效,可改善腹痛程度及中医证候。Objective To observe the clinical effects of Wenjinghuayu Treatment on primary dysmenorrhea of the type of Hanning blood stasis, regarding immediate pain relief as the primary efficacy endpoint. Methods Select the 90 cases with primary dysmen- orrhea of the type of Hanning blood stasis, divided randomly into three groups such as the impact of TCM group （30 cases with Xiang Yan Zhi Tong Prescription formula particles impact the administration）, TCM and acupressure group （30 cases with Xiang Yah Zhi Tong Prescription formula particles and acupressure routine administration）, western medicine group （30 cases with ibu- profen）. Three months of continuous treatment are cycle, observe and compare the clinical effects of the three groups before and after the treatment. Results The duration of pain in the impact of TCM group and the TCM and acupressure group was reduced while the TCM syndromes were improved, which were superior to those in the western medicine group （P 〈 0.05）. The abdomi- nal pain effect and the immediate pain relief time were significantly improved and the differences of the three groups were not sta- tistically significant （ P 〉 0.05 ）. The abdominal pain efficacy and symptom score of three menstrual cycle follow - up after treat- ment in the impact of TCM group and the TCM and acupressure group were better than those in the western medicine group （P 〈 0.01 ）. Conclusion The shock therapy of Xiang Yan Zhi Tong Prescription and TCM and acupressure method on Hanning stasis syndrome of primary dysmenorrhea have a significant effect, can improve the degree of pain and TCM syndrome and improve the o- verall efficiency of the treatment.
