摘 要:目的应用肌电生物反馈疗法与步态训练矫正仪治疗脑卒中偏瘫足下垂患者,观察足下垂的改善效果。方法86例脑卒中偏瘫足下垂患者随机分为对照组与研究组各43例,对照组应用步态训练矫正仪进行治疗,研究组应用肌电生物反馈疗法进行治疗;治疗2个疗程后,检测两组患者治疗前后步行改善情况,并应用Berg平衡量表(BBS)评估患者平衡能力、Fugl-Meyer运动功能量表评估患侧下肢运动功能,Holden步行功能分级评价步行能力。结果治疗后,研究组患者Brunnstrom分级优于对照组;踝背伸ROM和10m步行速度高于对照组;"起立—行走"计时低于对照组,差异均有统计学意义(P<0.05)。研究组患者治疗后BBS、Ful-meyer评分和Holden分级分别为39.50±6.92分、30.52±3.96分和3.12±0.71级,均显著高于对照组(P<0.05)。结论肌电生物反馈疗法与步态训练矫正仪治疗对于脑卒中偏瘫足下垂均具有较好的改善作用,应用肌电生物反馈疗法患者踝关节活动范围、步行速度等步行异常状态改善效果更明显。Objective To understand the effect of electromyography biofeedback therapy and that of gait correction instrument in treating foot drop as a result of stroke from hemiplegia. Method 86 patients with foot drop as result of stroke from hemiplegia were randomly divided into a control group( n = 43) and an experimental group( n = 43). The control group were treated with gait correction while the experimental group were treated with electromyography biofeedback therapy. After two sessions of treatment,the improvement in gait of the two groups was observed. Berg balance scale( BBS) was used to assess the subjects' balancing capacity. Fugl-Meyer Motor function was used to assess the lower extremity motor function of the subjects. Holden walk functional classification was used to assess their walking ability.Findings Brunnstrom grade of the experimental group after treatment was significantly better than that of the control group.The ankle dorsiflexion ROM and 10 m walking speed( 10 m MWS) of the experimental group after treatment were lower than that of the control group. " stand- walking" timing of the experimental group after treatment was lower than that of the control group. The differences were statistically significant( P〈0. 05). BBS,Ful- meyer rates and Holden rates of theexperimental group after treatment were( 39. 50 ± 6. 92) points,( 30. 52 ± 3. 96) points and( 3. 12 ± 0. 71) points respectively,higher than those of the control group. The difference was statistically significant( P〈0. 05). Conclusion Electromyography biofeedback therapy and gait correction apparatus were both found effective in the treatment of foot drop as a result of stroke from hemiplegia,with electromyography bio- feedback therapy being more effective in improving ankle joint range of motion,walking speed,and manner of walking.
