尿沉渣分析仪检测小圆上皮细胞的可靠性及参考值建立

摘　　要：目的分析影响尿沉渣分析仪检测小圆上皮细胞的影响因素。建立唐山市区正常人群小圆上皮细胞的参考区间。方法收集2015年11月-2016年2月住院和门诊患者尿沉渣标本2 403份,使用UF-1000i尿沉渣分析仪分析和显微镜镜检,对数据进行统计得出尿分析仪的准确率,分析导致小圆上皮细胞检测错误的影响因素;随机收集体检健康人群尿液标本1 200份,确立仪器法95%尿沉渣小圆上皮细胞参考区间。结果 UF-1000i尿流式细胞仪检测小圆上皮细胞的假阳性率为18.35%,假阴性率为0.84%。导致假阳性的主要影响因素为尿中的白细胞、形态较小的表面上皮细胞和滴虫等。对含有以上3种细胞的尿液标本中的小圆上皮细胞数据进行统计,表明分析仪和镜检结果的差异均有统计学意义(t值分别为6.41、3.23、5.43,P<0.05)。唐山市区尿沉渣小圆上皮细胞的参考区间:男性为0个/μl^3.40个/μl:女性为0个/μl^3.64个/μl。结论 UF-1000i的准确性较高,其中假阴性率极低,但假阳性率较高,导致假阳性的主要因素为白细胞等细胞增高引起的分析仪误判,因此临床检测时遇到白细胞等细胞增高的标本要进行显微镜镜检。Objective To study the influence factors of urinary sediment analyzer on small round epithelial cells,and to establish the normal reference range in Tangshan residents. Methods 2 403 urinary sediment test specimens from outpatients and hospitalized patients were randomly collected during November 2015-January 2016 for analysis with UF-1000 i urinary sediment analyzer and microscopic method; the data were summarized to obtain the accuracy of urine sediment analyzer and the influence factors which resulted in detection error of small round epithelial cells; 1 200 physical examination normal urine sediments were collected to establish the 95% local reference range of small round epithelial cells. Results For UF-1000 i urinary sediment analyzer,the false positive rate of small round epithelial cells was 18. 35%,and the false negative rate was 0. 84%.The main influence factors that lead to false positive factors were white blood cells in urine,the epithelial cells of the small and trichomoniasis,etc. The data of small round epithelial cells containing more than 3 kinds of cells in urine samples were detected,and the difference was statistically significant between analyzer and microscopy results（ t = 6. 41,3. 23,5. 43,P 0. 05）.The reference range of small round epithelial cells of urinary sediment in Tangshan were men within 0-3. 40 and women within0-3. 64. Conclusion UF-1000 i accuracy is higher,the false negative rate is very low,but the false positive rate is higher,the major influence factor resulting in false positive results is the increase of white blood cells,and thus,it needs microscopic examination for specimens with white blood cells increase.
