摘　　要：目的分析我院2008年1月至2014年1月6年期间妊娠期急性脂肪肝患者的临床资料、病例特点，为妊娠期急性脂肪肝的临床诊治提供可靠依据。方法收集2008年1月至2014年1月于我院妇产科住院诊治的3例妊娠急性脂肪肝患者的临床资料，包括临床表现、血清学检查、B超检查、诊治、转归、并发症等资料，对上述临床资料进行分析。结果3例患者均为初产妇，发病孕周36-38周，均有不同程度的恶心、呕吐和黄疸，1例患者有精神症状合并产后出血。3例患者均有肝功异常，2例有肾功异常、低血糖、血小板降低和凝血功能障碍。3例均为剖宫产术终止妊娠，产妇均存活。1例胎儿有胎儿窘迫症状，经救治存活。结论妊娠期急性脂肪肝患者一旦确诊，应在综合治疗基础上尽快终止妊娠，多学科持续综合治疗是成功救治的关键。Objective We analyzed the clinical features of acute fatty liver of pregnancy （AFLP） to provide clinical evidence for its diagnosis and therapy. Methods We collected the clinical data of the 3 cases with AFLP who were treated in Beijing Shijitan Hospital from January 2008 to January 2014. Clinical manifestations, serologic test results, B ultrasonic examination results, diagnosis and treatment outcome, and complications were all analyzed retrospectively. Results 3 patients were all primipara, gestational age 36 to 38 weeks, with varying degrees of nausea, vomiting and jaundice, 1 cases with mental symptoms and postpartum hemorrhage,3 patients with abnormal liver function, 2 cases with abnormal renal function, low blood sugar, low blood platelet and blood coagulation dysfunction. Three fetuses were survived by cesarean section. One fetus had distress symptoms, who survival by the treatment. Conclusion Once the patient is diagnosed as acute fatty liver of pregnancy, the pregnancy should be terminated as soon as possible as the pregnant women receive the comprehensive treatments. The key to success in rescue of the pregnant women was the combined multidisciplinary therapy.
